To date, few Norwegian clinical ethics committees (CECs) have included patients or next of kin in case discussions. In 2008, Rikshospitalet's (The National Hospital's) CEC began to routinely invite patients and relatives into case discussions. In this paper, we describe seven cases discussed by this committee in 2008. Six involved life and death decision-making in collaboration with the next of kin, while one related case did not include relatives. In our opinion, representing the patient's perspective was advantageous to the discussion (...) itself, to the conclusion made and to the next of kin's acceptance of the resolution. We believe that if the patient had been represented in the last case, the outcome might have been different. We conclude that successful patient involvement will rely on well-structured case discussions, an open atmosphere and good preparation and follow-up. (shrink)

The structure of ethics work in a hospital is complex. Professional ethics, research ethics and clinical ethics committees (CECs) are important parts of this structure, in addition to laws and national and institutional codes of ethics. In Norway all hospital trusts have a CEC, most of these discuss cases by means of a method which seeks to include relevant guidelines and laws into the discussion. In recent years many committees have received more cases which have concerned questions of principle. According (...) to Ellen Fox and co-authors the traditional CEC model suffers from a number of weaknesses. Therefore, in their organization a separate body deals with organizational matters. In this paper, we discuss what is gained and what is lost by creating two separate bodies doing ethics consultation. We do this through an analysis of detailed minutes of CEC discussions in one CEC during a 6-year period. 30 % of all referrals concerned matters of principle. Some of these discussions originated in a dilemma related to a particular patient. Most of the discussions had some consequences within the hospital organization, for clinical practice, for adjustment of guidelines, or may have influenced national policy. We conclude that a multiprofessional CEC with law and ethics competency and patient representation may be well suited also for discussion of general ethical principles. A CEC is a forum which can help bridge the gap between clinicians and management by increasing understanding for each others’ perspectives. (shrink)

This ambitious book presents a new interpretation of Chinese thought guided both by a philosopher's sense of mystery and by a sound philosophical theory of meaning. This chapter argues that explicit self-conscious thinking is founded on an implicit form of self-awareness built into the very structure of phenomenal consciousness. In broad strokes, the argument is that a theory denying the existence of pre-reflective or minimal self-awareness has difficulties explaining a number of essential features of explicit first-person self-reference, and that this will impede a proper understanding of certain types of psychopathology. The chapter proceeds by discussion of a number of prominent theories of self-knowledge and self-reference relating (...) them to forms of self-consciousness. The following article presents preliminary reflections on a processual theory of health and disease. It does this by steering the discussion more toward an ontology of organisms rather than conceptual analysis of the semantic content of the terms “health” and “disease.” In the first section, four meta-theoretical assumptions of the traditional debate are identified and alternative approaches to the problems are presented. Afterwards, the view that health and disease are constituted by a dynamic relation between demands imposed on an organism (...) and individual presuppositions for adequate response is developed. In the last section, the paper takes stock of three possible objections to and clarifies some implications of this approach to the notions of health and disease. (shrink)

The sense of agency is typically defined as the experience of controlling one’s own actions, and through them, changes in the external environment. It is often assumed that this experience is a single, unified construct that can be experimentally manipulated and measured in a variety of ways. In this article, we challenge this assumption. We argue that we should acknowledge four possible agency-related psychological constructs. Having a clear grasp of the possible constructs is important since experimental procedures are only able (...) to target some but not all the possible constructs. The unacknowledged misalignment of the possible constructs of a sense of agency and the experimental procedures is a major theoretical and methodological obstacle to studying the sense of agency. Only if we recognize the nature of this obstacle will we be able to design the experimental paradigms that would enable us to study the responsible computational mechanisms. (shrink)

Social interactions of autistic and non-autistic persons are intriguing. In all sorts of situations people with autism are part of the daily life of those around them. Such interactions exist despite the lack of familiar ways of attuning to one another. In Autistic Company, the anthropologist and philosopher Ruud Hendriks—himself trained as a care worker for young people with autism—investigates what alternative means are sometimes found by autistic and non-autistic people to establish a shared existence. It is generally assumed that storing predictive relations between two events in memory as bi-directional associations does not require conscious awareness of this relation, whereas the formation of unidirectional associations that capture the direction of the relation does. This study reports a set of experiments demonstrating that unidirectional associations can be formed even when awareness of the relation is actively prevented, if attention is “tuned” to process predictive relations. When participants engaged in predicting targets based on cues in an unrelated (...) task before the actual acquisition phase, unidirectional associations were formed during this acquisition phase even though E1 was presented subliminally. This suggests that although processing the relation between events may often be accompanied by awareness of this relation, awareness is not a prerequisite for the formation of unidirectional associations. When a person decides to do something in the future, she forms an intention and her intention persists. Philosophers have thought about the rational requirement that an agent’s intention persists until its execution. But philosophers have neglected to think about the causal memory mechanisms that could enable this kind of persistence and its role in rational long-term agency. Our aim of this paper is to fill this gap by arguing that memory for intention is a specific kind of memory. We (...) do this by evaluating and rejecting standard declarative accounts of memory for intention and arguing for the plausibility of an alternative model of memory for intention. We argue for the alternative by spelling out a number of computational principles that could enable retaining and retrieving intentions from long-term memory. During the past decades, research collaboration between researchers from different disciplines has become more frequent. However, there is a need to look into the generic modalities and challenges. The article explores a series of potential obstructions to cross-disciplinary collaboration of methodological and epistemological nature. Furthermore, a number of contextual, inhibiting factors are outlined. As means of overcoming the obstacles, the importance of mutual knowledge, allocation of adequate time and conducive research management is emphasised. New teams may benefit from tutoring by (...) facilitators, who can help to make problem areas explicit and negotiate solutions. Owing to the training background of the author, most of the examples are drawn from the interface between biomedicine/natural science and applied medical anthropology. However, the issues raised basically apply to all sorts of cross-disciplinary research collaboration in various combinations. (shrink)

Three experiments examined whether the mere priming of potential action effects enhances people’s feeling of causing these effects when they occur. In a computer task, participants and the computer independently moved a rapidly moving square on a display. Participants had to press a key, thereby stopping the movement. However, the participant or the computer could have caused the square to stop on the observed position, and accordingly, the stopped position of the square could be conceived of as the potential effect (...) resulting from participants’ action of pressing the stop key. The location of this position was primed or not just before participants had to stop the movement. Results showed that priming of the position enhanced experienced authorship of stopping the square. Additional experimentation demonstrated that this priming of agency was not mediated by the goal or intention to produce the effect. (shrink)

In this paper, we propose a new mathematical model of retrieval of intentions from long-term memory. We model retrieval as a stochastic race between a plurality of potentially relevant intentions stored in long-term memory. Psychological theories are dominated by two opposing conceptions of the role of memory in temporally extended agency – as when a person has to remember to make a phone call in the afternoon because, in the morning, she promised she would do so. According to the Working (...) Memory conception, remembering to make the phone call is explained in terms of the construction and maintenance of intentions in working-memory. According to the Long-Term Memory conception, we should explain the episode in terms of an ability to store intentions in long-term memory. The two conceptions predict different processing profiles. The aim of this paper is to present a new mathematical model of the type of memory mechanism that could realise the long-term memory representations of intentions necessary for the Long-Term Memory conception. We present and illustrate the formal model and propose a new type of experimental paradigm that could allow us to test which of the two conceptions provides the best explanation of the role of memory in temporally extended agency. The objective of this research project is to develop a preliminary examination of an heuristic process ontology derived from an east-west comparative methodology. It attempts to trace the similarities and discontinuities of an ontological perspective in Friedrich Nietzsche‘s philosophy and several different strands of thought in Warring States era Chinese philosophical thought, focusing on Daoism in particular. The project traces the conclusions of these comparisons from a basic theoretical ontology to a socio-practical consideration. It concludes that in theorizing process both (...) perspectives do not rely on traditional dichotomies that are seen in Western philosophical thought, they see the world as non-deterministic and utilize correlative thinking. The research traces further considerations in the areas of epistemology and evaluation based on these points and concludes that there is no separation between epistemology-evaluation and the underlying ontology, they are direct continuations of ontology. As a last question of theory, this research examines the consequences of comparative process ontology for language, claiming that it allows us to undermine a subjective/objective dichotomy by naturalizing language. Lastly, the theoretical groundwork of this project is applied to a number of extant philosophical issues. It attempts to resolve the dichotomy of reality and appearance as a metaphysical issue, and offers an account of how socio-political and economic issues can be theorized according to such an ontology. Ending world hunger remains one of the central global challenges, but the question of how to measure and define the problem is politically charged. This article chronicles and analyses the indicator selection process for SDG 2.1, focusing in particular on the Food Insecurity Experience Scale (FIES) indicator. Despite alleged efforts to separate political and technical aspects in the indicator selection process we find that they were entangled from the start. While there was significant contestation around which indicators should be selected, (...) the process was characterized by pathway lock-in: The complexity of food security quantification and the resource constraints in the process favored already established data infrastructures and milieus of expertise, locking in the position of FAO and its established food security indicators. The SDG 2.1 indicators frame food insecurity in terms of caloric supply and demand and individual experience, arguably excluding dimensions of democratic agency, sustainability and other dimensions and drivers of food insecurity. The lock-in has thus embedded a narrow concept of food security in the major global indicator framework for food security monitoring. This is likely to have significant effects on how food insecurity is addressed nationally and internationally. Addressing the knowledge politics of food security indicators is important to broaden and open the agenda for sustainable transformation of food systems. Statistics and indicators are important tools in this agenda, but a diversity of approaches and data infrastructures from the local to the international level are needed to understand the multiple dimensions and drivers of food insecurity. The aim of this essay is to reflect on the place and importance of the question of address and to show how it comes to the fore in Søren Kierkegaard’s writings. What shall be attempted, with regard to Kierkegaard’s already widely recognized renown as an existential thinker, is to catch a glimpse of issues that make up the larger background in which the question of address is embedded. In doing so, the essay explores several features of Kierkegaard’s inquiry into the (...) question of existence in the Concluding Unscientific Postscript and connect them with the notion of the reader. This latter notion provides a way of understanding the singularity of voice, while offering an account of the testimony to existence. After a brief recapitulation, the essay identifies in conclusion unresolved issues relating to speech, silence, confidentiality, and listening. (shrink)

Psychologists and philosophers are often tempted to make general claims about the importance of certain experimental results for our commonsense notions of intentional agency, moral responsibility, and free will. It is a strong intuition that if the agent does not intentionally control her own behavior, her behavior will not be an expression of agency, she will not be morally responsible for its consequences, and she will not be acting as a free agent. It therefore seems natural that the interest centers (...) on the notion of intentional control. If it can be experimentally shown that agents do as a matter of fact not control their own actions, even though they think they do, it will have far reaching consequences for our moral psychology. In this paper I look at recent eliminative arguments allegedly demonstrating that our commonsense notion of intentional control is incompatible with experimental data in support of the dual visual stream theory. Gilles de la Tourette syndrome (TS) is a childhood neuropsychiatric disorder characterised by the presence of motor and vocal tics. Patients with malignant TS experience severe disease sequelae; risking morbidity and mortality due to tics, self-harm, psychiatric comorbidities and suicide. By definition, those cases termed ‘malignant’ are refractory to all conventional psychiatric and pharmacological regimens. In these instances, deep brain stimulation (DBS) may be efficacious. Current 2015 guidelines recommend a 6-month period absent of suicidal ideation before DBS is offered to (...) patients with TS. We therefore wondered whether it may be ethically justifiable to offer DBS to a minor with malignant TS. We begin with a discussion of non-maleficence and beneficence. New evidence suggests that suicide risk in young patients with TS has been underestimated. In turn, DBS may represent an invaluable opportunity for children with malignant TS to secure future safety, independence and fulfilment. Postponing treatment is associated with additional risks. Ultimately, we assert this unique risk-benefit calculus justifies offering DBS to paediatric patients with malignant TS. A multidisciplinary team of clinicians must determine whether DBS is in the best interest of their individual patients. We conclude with a suggestion for future TS-DBS guidelines regarding suicidal ideation. The importance of informed consent and assent is underscored. (shrink)

This study investigates to what extent the amount of variation in a visual scene causes speakers to mention the attribute color in their definite target descriptions, focusing on scenes in which this attribute is not needed for identification of the target. The results of our three experiments show that speakers are more likely to redundantly include a color attribute when the scene variation is high as compared with when this variation is low (even if this leads to overspecified descriptions). We (...) argue that these findings are problematic for existing algorithms that aim to automatically generate psychologically realistic target descriptions, such as the Incremental Algorithm, as these algorithms make use of a fixed preference order per domain and do not take visual scene variation into account. Medically unexplained symptoms (MUS) remain recalcitrant to the medical profession, proving less suitable for homogenic treatment with respect to their aetiology, taxonomy and diagnosis. While the majority of existing medical research methods are designed for large scale population data and sufficiently homogenous groups, MUS are characterised by their heterogenic and complex nature. As a result, MUS seem to resist medical scrutiny in a way that other conditions do not. This paper approaches the problem of MUS from a philosophical point of (...) view. The aim is to first consider the epistemological problem of MUS in a wider ontological and phenomenological context, particularly in relation to causation. Second, the paper links current medical practice to certain ontological assumptions. Finally, the outlines of an alternative ontology of causation are offered which place characteristic features of MUS, such as genuine complexity, context-sensitivity, holism and medical uniqueness at the centre of any causal set-up, and not only for MUS. This alternative ontology provides a framework in which to better understand complex medical conditions in relation to both their nature and their associated research activity. (shrink)

Concern for and interest in research integrity has increased significantly during recent decades, both in academic and in policy discourse. Both in terms of diagnostics and in terms of therapy, the tendency in integrity discourse has been to focus on strategies of individualisation (detecting and punishing individual deviance). Other contributions to the integrity debate, however, focus more explicitly on environmental factors, e.g. on the quality and resilience of research ecosystems, on institutional rather than individual responsibilities, and on the quality of (...) the research culture. One example of this is the Bonn PRINTEGER Statement. This editorial to the LSSP thematic series (article collection) entitled Addressing integrity challenges in research: the institutional dimension invites authors to contribute to the research integrity debate. Notably, we are interested in submissions addressing issues such as institutional responsibilities, changes in the research climate, duties of research managers and research performing or research funding organisations (RPOs and RFOs) as well as new approaches to integrity education. Phenomenology of illness has grown increasingly popular in recent times. However, the most prominent phenomenologists of illness defend a psychologizing notion of phenomenology, which argues that illness is primarily constituted by embodied experiences, feelings, and emotions of suffering, alienation etc. The article argues that this gives rise to three issues that need to be addressed. (1) How is the theory of embodiment compatible with the strong distinction between disease and illness? (2) What is the difference between problematic embodiment and illness? (...) (3) How is existential edification, that illness can give rise to according to the phenomenologists, to be understood? The article then engages in an analysis of Heidegger’s and Waldenfels’ phenomenology with the ambition of developing a notion of existence, which can transgress the psychologization of illness. Rather than arguing that illness is constituted by experiences of suffering and alienation, it emphasizes that broaches upon conatively guided activities constitute illness. (shrink)

In two experiments, we investigate to what extent various visual saliency cues in realistic visual scenes cause speakers to overspecify their definite object descriptions with a redundant color attribute. The results of the first experiment demonstrate that speakers are more likely to redundantly mention color when visual clutter is present in a scene as compared to when this is not the case. In the second experiment, we found that distractor type and distractor color affect redundant color use: Speakers are most (...) likely to overspecify if there is at least one distractor object present that has the same type, but a different color than the target referent. Reliable effects of distractor distance were not found. Taken together, our results suggest that certain visual saliency cues guide speakers in determining which objects in a visual scene are relevant distractors, and which not. We argue that this is problematic for algorithms that aim to generate human-like descriptions of objects, since these generally select properties that help to distinguish a target from all objects that are present in a scene. In recent decades there has been increasing demand for and considerable efforts to conduct cross-disciplinary research. However, assessment of research quality in such endeavours still is often based on mono-disciplinary criteria and not seldom carried out by reviewers without strong cross-disciplinary experience. The authors suggest a two-pronged approach to cross-disciplinary research evaluation. One part should comprise an individual review of all the disciplines involved based on their mono-disciplinary sets of criteria. The other part should be a separate evaluation of the (...) cross-disciplinary aspects based on the review of “problem formulation”, “integration and scope of the disciplines”, “parts and the whole”, “practical managerial aspects” and “the applied aspects”. The pros and cons of implementing this approach in a stepwise manner or simultaneously is discussed. It is suggested that funding agencies develop more fair sets of review procedures for cross-disciplinary research and show willingness to allocate extra funds and time to such forms of research that sometimes are regarded as relatively more “risky” than conventional mono-disciplinary types. The epidemiology is confusing, different medical specialties claim ownership of their unexplained territory and the unexplained conditions are themselves promoted through a highly complicated and sophisticated use of language. Confronting the outcome, i.e. numerous medical acronyms, we reflect upon principles of systematizing, contextual and social considerations and ways of thinking about these phenomena. Finally we address what we consider to be crucial dimensions concerning the landscape of unexplained “matters”; fatigued (...) being, pain-full being and dys-ordered being, all expressive momentums of an aesthetic of resistance. There is one concept in medicine which is prominent, the symptom. The omnipresence of the symptom seems, however, not to be reflected by an equally prominent curiosity aimed at investigating this concept as a phenomenon. In classic, traditional or conventional medical diagnostics and treatment, the lack of distinction with respect to the symptom represents a minor problem. Faced with enigmatic conditions and their accompanying labels such as chronic fatigue syndrome, fibromyalgia, medically unexplained symptoms, and functional somatic syndromes, the contestation of (...) the symptom and its origin is immediate and obvious and calls for further exploration. Based on a description of the diagnostic framework encompassing medically unexplained conditions and a brief introduction to how such symptoms are managed both within and outside of the medical clinic, we argue on one hand how unexplained conditions invite us to reconsider and re-think the concept we call a “symptom” and on the other hand how the concept “symptom” is no longer an adequate and necessary fulcrum and must be enriched by socio-cultural, phenomenological and existential dimensions. Consequently, our main aim is to expand both our interpretative horizon and the linguistic repertoire in the face of those appearances we label medically unexplained symptoms. (shrink)
