I came to utopia by accident, having painted myself into a corner with an idea for a trilogy: three science fiction novels consisting of an after-the-fall novel, a dystopia, and a utopia, all set in the same place and about the same distance into the future. Early in the COVID-19 pandemic there was widespread concern that healthcare systems would be overwhelmed, and specifically, that there would be insufficient critical care capacity in terms of beds, ventilators or staff to care for patients. In the UK, this was avoided by a threefold approach involving widespread, rapid expansion of critical care capacity, reduction of healthcare demand from non-COVID-19 sources by temporarily pausing much of normal healthcare delivery, and by governmental and societal responses that reduced demand through national lockdown. (...) Despite high-level documents designed to help manage limited critical care capacity, none provided sufficient operational direction to enable use at the bedside in situations requiring triage. We present and describe the development of a structured process for fair allocation of critical care resources in the setting of insufficient capacity. The document combines a wide variety of factors known to impact on outcome from critical illness, integrated with broad-based clinical judgement to enable structured, explicit, transparent decision-making founded on robust ethical principles. It aims to improve communication and allocate resources fairly, while avoiding triage decisions based on a single disease, comorbidity, patient age or degree of frailty. It is designed to support and document decision-making. The document has not been needed to date, nor adopted as hospital policy. However, as the pandemic evolves, the resumption of necessary non-COVID-19 healthcare and economic activity mean capacity issues and the potential need for triage may yet return. Systematic study of the intersection of ethics consultation services and solid organ transplants and recipients can identify and illustrate ethical issues that arise in the clinical care of these patients, including challenges beyond resource allocation. This was a single-centre, retrospective cohort study of all adult ethics consultations between January 1, 2007, and December 31, 2017, at a large academic medical centre in the north-eastern United States. Of the 880 ethics consultations, sixty (6.8 per cent ) involved solid organ transplant, thirty-nine (...) (65.0 per cent) for candidates and twenty-one (35.0 per cent ) for recipients. Ethics consultations were requested for 4.3 per cent of heart, 4.9 per cent of lung, 0.3 per cent of liver, and 0.3 per cent of kidney transplant recipients over the study period. Nurses were more likely to request ethics consultations for recipients than physicians (80.0 per cent vs 20.0 per cent, p = 0.006). The most common reason for consultation among transplant candidates was discussion about intensity of treatment or goals of care after the patient was not or was no longer a transplant candidate. The most common reason for ethics consultation among transplant recipients was disagreement between transplant providers and patients/families/non-transplant healthcare professionals over the appropriate intensity of treatment for recipients. Very few consultations involved questions about appropriate resource allocation. Ethics consultants involved in these cases most often navigated communication challenges between transplant and non-transplant healthcare professionals and patients and families. (shrink)

In the United States, targeted school shootings have become a distinct genre of violence. In this essay, Bryan Warnick, Sang Hyun Kim, and Shannon Robinson examine the social meanings that exist in American society that might contribute to this phenomenon, focusing on the question: “Why are schools conceptualized as appropriate places to enact this form of gun violence?” The authors analyze the social meaning of American schooling by using empirical data, everyday observations, films, and poetry, and then connect these (...) points of meaning to stories of individual school shooters. Through this analysis, three aspects of school stand out. First, schools are places of both real and symbolic violence, where force and power often rule the day. Second, schools are places connected to expectations of hope and refuge, friendship and romance, and when these expectations are not met, bitter resentment flows against schools. Third, suburban schools are seen as places of expressive individualism, which, in rare cases, is manifest in terms of “expressive violence.” Together, these points of meaning can make schools, for some youth, seem like appropriate places to express violent intentions. This research attempts to evaluate the hermeneutic characteristics of catuṣkoṭi (tetralemma) in the ‘Nirvāṇa’ Chapter of the _Mūlamadhyamakakārikā_ (Ch. 25), focusing on the identity thesis between nirvāṇa and saṃsāra. Regarding the structure of the tetralemma posited by Nāgārjuna (ca. 150-ca. 250), this study criticizes the dialectical interpretation of Robinson and Kajiyama from the perspective of Siderits and Katsura’s semantic approach to the extent that it does not deny ultimate truth. This sets it apart from the semantic view presented by (...) Siderits and Garfield (J Indian Philos 41:655–664, 2013). To support this position, the commentaries of Piṅgala (ca. 4C) and Candrakīrti (ca. 600–ca. 650) are also used, along with Nāgārjuna’s _Vigrahavyāvartanī_. From this standpoint, the meaning of non-duality of nirvāṇa and saṃsāra is evaluated, in the sense that both are negated in the ultimate sense, as opposed to the metaphysical view, which interprets the tetralemma in a dialectical way. Siderits and Katsura suggests that, as in the case of nirvāṇa, “just as the four lemmas applying to saṃsāra is rejected, ultimately there is no such state as saṃsāra,” because it is empty of its intrinsic nature. Following this line of reasoning, the thesis of Tathāgata in 25.17 and 25.18 can also be interpreted semantically as “there is no real distinction between before and after death, due to the absence of intrinsic nature (_svabhāva_) in Tathāgata.” Likewise, the semantic approach here negates the four propositions of nirvāṇa and saṃsāra only, as operated when clinging to identity. In Kim Stanley Robinson, part of the Modern Masters of Science Fiction series published by the University of Illinois Press, Robert Markley ambitiously sets out to tell the story of Kim Stanley Robinson's oeuvre by offering a "mid-career report that traces the developing concerns in his fiction". The challenge of connecting the wide-ranging concerns of Robinson's fiction necessarily risks eliding or simplifying key elements of that story. Nonetheless, Markley is able to leverage Robinson's characteristic iteration over (...) crucial narrative and critical themes throughout his oeuvre to articulate the trajectory of Robinson's concerns across three decades of writing, making this work an excellent introduction to a... Kim Stanley Robinson is a declared eco-socialist and arguably the most distinguished acolyte of Fredric Jameson, America's leading Marxist literary critic. Robinson's Mars trilogy develops a detailed account of three political revolutions. Robinson explains that this was a deliberate choice on his part, because he felt that in his first utopian novel, Pacific Edge, he “dodged the necessity of revolution.” He describes Antarctica and The Years of Rice and Salt as his next utopian novels, but these, too, (...) dodge the “necessity of revolution,” the first by substituting science for politics, the second by projecting an alternative history into an alternative future. In the Science in the Capital trilogy, politics becomes paramount, and it continues to be so in his more recent fiction. This article examines the utopianism of Kim Stanley Robinson's 2017 sf novel New York 2140, the world of which is defined by a nexus of profound climatological disasters. In contrast to other texts in the growing cli-fi genre, Robinson's seemingly apocalyptic world is a representation of a disaster utopia—real-world networks of care, compassion, collaboration, and utopian joy that emerge in the wake of disasters. In New York 2140 Robinson explores the valences of disaster utopianism beyond its contemporary, (...) exceptional appearances, contemplating how communities can flourish in a world where disaster emerges as a direct result of capitalist precarity and inequality. The article situates New York 2140 within a newly emerging corpus of cultural work that calls for a direct confrontation with the destructive powers of the present by understanding utopia as a precarious and never-finished form of resistance to neoliberal late capitalism. It envisages an intercultural transmission of religious and philosophical ideas from India to China.

For at least 50 years science fiction’s dangerousness has sprung largely from its leaps into the transgressive. But something has now changed; the biggest problem today for anyone trying to create dangerous science fiction is that in the developed countries we now live largely in a libertarian, post-transgressive culture. There is, however, at least one target for science fiction that grows increasingly dangerous; the border between scarcity and post-scarcity. This danger is perhaps best realized in the great Mars trilogy by (...) Kim Stanley Robinson, which not only imagines a bridge from our current scarcity-based culture to a post-scarcity culture, but also shows people building this very bridge up from the foundations of our life today. Perhaps more dangerously, he then follows Marcuse and Mumford in envisioning not only an end to the economic problem of poverty but also to the social, anthropological and psychical forces of scarcity, showing step-by-step the dramatic changes in psychology, values and lives that post-scarcity would bring. And so, like certain tales by Plato and Tolstoy, it is a ‘threshing tale’, a tale that forces us into an imaginative confrontation with our current values, intending to winnow the false from the true. This epic sets out to alter the way we see ourselves and our social sphere, hoping – like William Blake – that altering the eye alters all; perhaps nothing but such a plausible, non-utopian and social vision of life in post-scarcity can be truly dangerous to the way we now think, love, work and war. Direction of fit theories usually claim that beliefs are such that they “aim at truth” or “ought to fit” the world and desires are such that they “aim at realization” or the world “ought to fit” them. This essay argues that no theory of direction of fit is correct. The two directions of fit are supposed to be determinations of one and the same determinable two-place relation, differing only in the ordering of favored terms. But there is no such determinable (...) because of ineliminable asymmetries between the way that beliefs “aim at truth” and the way that desires “aim at realization.” This essay traces the ills of direction of fit theory to a misunderstanding of Anscombe and proposes a cure that distinguishes theoretical and practical thought by appeal to a distinction between thought in the form of a state and thought in the form of an event. Is Anscombean practical knowledge independent of what the agent actually does on an occasion? Failure to understand Anscombe’s answer to this question is a major obstacle to appreciating the subtlety and plausibility of her view. I argue that Anscombe’s answer is negative, and turns on the nature of mistakes in performance, and reveals a distinctive implicit metaphysics of mind and knowledge, structured by related capacities and exercises of capacities. If my interpretation is correct, then practical knowledge shares features with knowledge-how (...) and knowledge-that, but deserves its own epistemic category. (shrink)

Responsible leadership is rare. It is not that most leaders are irresponsible, but responsibility in leadership is frequently defined so that an important connotation of responsible leadership is ignored. This article equates responsible leadership with virtuousness. Using this connotation implies that responsible leadership is based on three assumptions—eudaemonism, inherent value, and amplification. Secondarily, this connotation produces two important outcomes—a fixed point for coping with change, and benefits for constituencies who may never be affected otherwise. In men and women sexual arousal culminates in orgasm, with female orgasm solely from sexual intercourse often regarded as a unique feature of human sexuality. However, orgasm from sexual intercourse occurs more reliably in men than in women, likely reflecting the different types of physical stimulation men and women require for orgasm. In men, orgasms are under strong selective pressure as orgasms are coupled with ejaculation and thus contribute to male reproductive success. By contrast, women's orgasms in intercourse are highly (...) variable and are under little selective pressure as they are not a reproductive necessity. The proximal mechanisms producing variability in women's orgasms are little understood. In 1924 Marie Bonaparte proposed that a shorter distance between a woman's clitoris and her urethral meatus (CUMD) increased her likelihood of experiencing orgasm in intercourse. She based this on her published data that were never statistically analyzed. In 1940 Landis and colleagues published similar data suggesting the same relationship, but these data too were never fully analyzed. We analyzed raw data from these two studies and found that both demonstrate a strong inverse relationship between CUMD and orgasm during intercourse. Unresolved is whether this increased likelihood of orgasm with shorter CUMD reflects increased penile–clitoral contact during sexual intercourse or increased penile stimulation of internal aspects of the clitoris. CUMD likely reflects prenatal androgen exposure, with higher androgen levels producing larger distances. Thus these results suggest that women exposed to lower levels of prenatal androgens are more likely to experience orgasm during sexual intercourse. . (shrink)

Promises to perform supererogatory actions present an interesting puzzle. On the one hand, this seems like a promise that one should be able to keep simply by performing some good deed or other. On the other hand, the only way to keep it is to do something that exceeds one’s duties. But any good deed that one performs, which might otherwise have been supererogatory, will not go above and beyond what one is morally required to do in such a case (...) because one has an obligation that one does not normally have—namely, an obligation to do something supererogatory. Thus, some scholars have argued that promises of this sort cannot possibly be kept and so are wrong to make. I show that, far from being impossible, keeping promises to supererogate is easy, and so there is nothing wrong with making promises of this sort. (shrink)

Confucian humility (qian xun 謙遜) is a deeply rooted virtue in East Asian traditions and widely practiced among modern East Asians. Despite its significance, our modern understanding of it remains imperfect, partly due to a prevailing misunderstanding of its true nature under the label of “modesty­bias.” This bias is often cited as a representative trait of East Asian collectivism in social or cultural psychology, leading to a narrow focus on attitudes and behaviors associated with it, with little attention to whether (...) it accurately reflects the historical roots of Confucian humility. This paper aims to highlight the notable differences between attitudes or behaviors related to modesty­bias and traditional Confucian humility, arguing that failing to make this distinction poses a significant obstacle to understanding Confucian humility as a virtue and its contemporary expression. Methodological suggestions are provided on how to conduct interdisciplinary research on Confucian humility, emphasizing the need to recognize and address the prejudice associated with modesty­bias. Such an interdisciplinary approach can also help discern modern attitudes or behaviors rooted in Confucian humility and shed new light on the continuing relevance of this traditional virtue in contemporary East Asian society. For the physicalist, the mind-body problem is the problem of finding a place for the mind in a world that is fundamentally physical. What does “fundamentally physical” mean? I think any physicalist will accept at least the following two claims. First, the world contains nothing but bits of matter and aggregates of bits of matter. There are no Cartesian souls, or Hegelian spirits, or neo-vitalist entelechies—as the emergentist C. Lloyd Morgan put it, no “alien influx” into the natural order. This (...) ontological thesis is sometimes called “ontological physicalism”. Second, we have the supervenience thesis: physical facts determine all the facts, and the physical properties of a thing determine all its properties. At this point, then, the world looks like this: all the things that exist are physical things—either basic bits of matter or wholly made up of bits of matter. These physical things have properties. What properties? First, there are basic physical properties, like mass, size, shape, electric charge, and so on—properties and magnitudes in terms of which laws of physics are formulated. But some properties of complex physical systems, like biological organisms, seem prima facie nonphysical—at least, they are not among the properties investigated in physics or the physical sciences. Prominent among them are mental properties—beliefs, desires, sensations, emotions, and the rest. The supervenience thesis says that such properties are fixed by the physical properties of the systems that have them; once the physical properties of a system are fixed, that fixes all of its properties. And yet property dualists, or nonreductive physicalists, maintain that although certain non-physical properties are determined by physical properties, they are irreducible to, and remain distinct from, physical properties. Although thoughts and pains are determined by the biology, and ultimately physicochemistry, of an organism, they are not biological/physical properties themselves, and there exists a special autonomous science of psychology, or cognitive science, that is responsible for investigating them. Some people of colour feel shame in response to racist incidents. This phenomenon seems puzzling since, plausibly, they have nothing to feel shame about. This puzzle arises because we assume that targets of racism feel shame about their race. However, I propose that when an individual is racialised as non-White in a racist incident, shame is sometimes prompted, not by a negative self-assessment of her race, but by her inability to choose when her stigmatised race is made salient. I argue (...) that this can make sense of some shame responses to racism. My account also helps to highlight some of the emotional and cognitive costs of racism that have their root in shame as well as a new form of hermeneutical injustice and distinctive communicative harms, contributing to a fuller picture of what is objectionable about racism. An observation of Hume’s has received a lot of attention over the last decade and a half: Although we can standardly imagine the most implausible scenarios, we encounter resistance when imagining propositions at odds with established moral (or perhaps more generally evaluative) convictions. The literature is ripe with ‘solutions’ to this so-called ‘Puzzle of Imaginative Resistance’. Few, however, question the plausibility of the empirical assumption at the heart of the puzzle. In this paper, we explore empirically whether the difficulty we (...) witness in imagining certain propositions is indeed due to claim type (evaluative v. non-evaluative) or whether it is much rather driven by mundane features of content. Our findings suggest that claim type plays but a marginal role, and that there might hence not be much of a ‘puzzle’ to be solved. (shrink)

Change is inevitable, and increasingly rapid and continuous in healthcare as organizations strive to adapt, improve and innovate. Organizational change challenges healthcare providers because it restructures how and when patient care delivery is provided, changing ways in which nurses must carry out their work. The aim of this doctoral study was to explore frontline nurses’ experiences of living with rapid and continuous organizational change. A critical hermeneutic approach was utilized. Participants described feeling voiceless, powerless and apolitical amidst rapid and continuous (...) organizational changes which fuelled apathy, cynicism and disengagement from the organization. However, critical analysis of the data showed that nurses actively engaged with power, voice and politics through resistant and transgressive behaviours in micro‐ethical moments of practice. There is a need to reconceptualize the concepts of voice, power and politics in nursing as there is dissonance between nurses’ beliefs about these concepts and what they are enacting in practice. Recognizing their enactment of power, voice and political agency at the micro‐level may empower nurses. Empowerment would mitigate the high levels of reports of powerlessness experienced in practice during organizational changes. (shrink)

This paper discusses two perspectives, each of which recognises the importance of environmental resources in enhancing and amplifying our cognitive capacity. One is the Clark–Chalmers model, extended further by Clark and others. The other derives from niche construction models of evolution, models which emphasise the role of active agency in enhancing the adaptive fit between agent and world. In the human case, much niche construction is epistemic: making cognitive tools and assembling other informational resources that support and scaffold intelligent action. (...) I shall argue that extended mind cases are limiting cases of environmental scaffolding, and while the extended mind picture is not false, the niche construction model is a more helpful framework for understanding human action.
