Background: Direct–to–consumer genetic testing (DTCGT) offers risk estimates for a variety of complex diseases and conditions, yet little is known about its impact on actual users, including their decisions about sharing the information gleaned from testing. Ethical considerations include the impact of unsolicited genetic information with variable validity and clinical utility on relatives, and the possible burden to the health care system if revealed to physicians. Aims: The qualitative study explored primary care patients’ views, attitudes, and decision making considerations regarding (...) DTCGT. This article focuses on the disclosure decisions participants made regarding participation, testing, and results of DTCGT, a topic which arose as a secondary aim of the study. Methods: Through four longitudinal interviews (pre–test, results, 3 and 12 months post–test) we examined twenty primary care patients’ decisions, expressed intentions, and actions regarding disclosure to immediate and extended family, friends and coworkers, and physicians about participation in and results of DTCGT. Individual interviews were analyzed using qualitative content analysis and a summative approach to describe the global themes. Results: Most participants disclosed to some immediate family; less than half disclosed to extended family; approximately half talked to friends. Most participants stated they would or might disclose to physicians about DTCGT and a few did. Conceptual themes that emerged from the data analysis include ambivalence about disclosure, consistency between intention and actual disclosure behavior and decisions, and conditional information sharing. Conclusions: Participants’ intentional and actual disclosure patterns offer insight into how they view DTCGT, weigh results, and the potential impact of DTCGT. (shrink)

I'd like to see a six-month program to include CPR and capillary blood glucose (CBG) training as well as more in depth anatomy training. I think six months would keep some, who are just looking for money, out of the field and help keep those of us who really want a career in nursing in the field. It would be a better stepping stone for those who plan to go on to a LPN or RN program.I currently have CBG training but my employer will not let me use it. It's disappointing because it's a skill I have that I can't use and it could really help out the nurses. I think if courses were a bit longer, CNAs might be taken a bit more seriously by their employers and respected by their colleagues in the medical field.I was trained in CBG while working for the hospital and received a card like a CPR card to show I was trained. I live and work and was trained in Arkansas. I'm not allowed to do CBGs in the long-term care and rehab facility where I work now, though. However, unless the resident is diabetic we do have to clip and file nails. It took many years for nurses to gain respect and be allowed to do some things that now CNAs do. For example, for many years doctors didn't feel nurses were qualified to even take vital signs. I suppose CNAs will have the same struggle. This essay seeks to show that teaching and learning are to be properly understood, not as an undertaking carried out on the will of a higher power or party, but as a way of life with an integrity of its own, arising from its own integral purposes. The essay thus seeks to provide an understanding of educational practice and of educational thought that contrasts in key respects with Alasdair MacIntyre's understanding, though also with a some notable parallels. A largely forgotten (...) ‘Socrates of Athens’ is identified as furnishing the original inspiration for the understanding of education explored in the essay. Some influential modern (and postmodern) negations of this understanding are then reviewed. Arising from its investigation of teaching and learning as a singular kind of relationship, the essay concludes with a brief sketch of some virtues that might constitute the way of life in question, in its more active and its more reflective moments. We are living in an algorithmic age where mathematics and computer science are coming together in powerful new ways to influence, shape and guide our behaviour and the governance of our societies. As these algorithmic governance structures proliferate, it is vital that we ensure their effectiveness and legitimacy. That is, we need to ensure that they are an effective means for achieving a legitimate policy goal that are also procedurally fair, open and unbiased. But how can we ensure that algorithmic (...) governance structures are both? This article shares the results of a collective intelligence workshop that addressed exactly this question. The workshop brought together a multidisciplinary group of scholars to consider barriers to legitimate and effective algorithmic governance and the research methods needed to address the nature and impact of specific barriers. An interactive management workshop technique was used to harness the collective intelligence of this multidisciplinary group. This method enabled participants to produce a framework and research agenda for those who are concerned about algorithmic governance. We outline this research agenda below, providing a detailed map of key research themes, questions and methods that our workshop felt ought to be pursued. This builds upon existing work on research agendas for critical algorithm studies in a unique way through the method of collective intelligence. (shrink)

Retrieval of relevant unstructured information from the ever-increasing textual communications of individuals and businesses has become a major barrier to effective litigation/defense, mergers/acquisitions, and regulatory compliance. Such e-discovery requires simultaneously high precision with high recall (high-P/R) and is therefore a prototype for many legal reasoning tasks. The requisite exhaustive information retrieval (IR) system must employ very different techniques than those applicable in the hyper-precise, consumer search task where insignificant recall is the accepted norm. We apply Russell, et al.’s cognitive task (...) analysis of sensemaking by intelligence analysts to develop a semi-autonomous system that achieves high IR accuracy of F1 ≥ 0.8 compared to F1 < 0.4 typical of computer-assisted human-assessment (CAHA) or alternative approaches such as Roitblat, et al.’s. By understanding the ‘Learning Loop Complexes’ of lawyers engaged in successful small-scale document review, we have used socio-technical design principles to create roles, processes, and technologies for scalable human-assisted computer-assessment (HACA). Results from the NIST-TREC Legal Track’s interactive task from both 2008 and 2009 validate the efficacy of this sensemaking approach to the high-P/R IR task. (shrink)

The current study examined the effects of cardiorespiratory fitness, identified with a continuous graded cycle ergometry, and aerobic exercise on cognitive functioning and entropy of the electroencephalogram (EEG) in 30 adolescents between the ages of 13 and 14 years. Higher and lower fit participants performed an executive function task after a bout of acute exercise and after rest while watching a film. EEG entropy, using the sample entropy measure, was repeatedly measured during the 1500ms post-stimulus interval to evaluate changes in (...) entropy over time. Analysis of the behavioural data for lower and higher fit groups revealed an interaction between fitness levels and acute physical exercise. Notably, lower fit, but not higher fit, participants had higher error rates for NoGo relative to Go trials in the rest condition, whereas in the acute exercise condition there were no differences in error rates between groups; higher fit participants also had significantly faster reaction times in the exercise condition in comparison with the rest condition. Analysis of EEG data revealed that higher fit participants demonstrated lower entropy post-stimulus than lower fit participants in the left frontal hemisphere, possibly indicating increased efficiency of early stage stimulus processing and more efficient allocation of cognitive resources to the task demands. The results suggest that EEG entropy is sensitive to stimulus processing demands and varies as a function of physical fitness levels, but not acute exercise. Physical fitness, in turn, may enhance cognition in adolescence by facilitating higher functionality of the attentional system in the context of lower levels of frontal EEG entropy. (shrink)

If Rome was for centuries the centre of power and influence for Christendom and the European world of learning associated with it, Brussels can claim to be such a twofold centre in the late twentieth century. The radical pluralism and postmodernist orientations which are now part of the Enlightenment legacy becloud the point that a new uniformity of belief and outlook—mercenary rather than spiritual—furnishes the context for most educational policy-making in European countries. Far from calling for a return to a (...) patriarchal past, the paper attempts to sketch an understanding of education as a universally defensible practice, while addressing the challenges of both postmodernism and the new uniformity. Australia is a suburban nation, with 85 percent of the 20 million people clinging to the coastal fringes of the world's largest island and oldest continent. This article explores Australian suburbia as the `third space' that mediates urbanism to `nature'. It draws on the thought of George Seddon, an important initiator of ecological history, regional geography and sub/urban politics in Australia. Seddon's insights on Australian ecosystems and Australian interpretations, namings, perceptions and shapings of their natural environment since the beginning of (...) European colonization in 1788 are used to think about the `nature of suburbia', `nature in suburbia' and suburbia in Australian `nature'. Its central argument is that nature is always culturally mediated and that the dominant ecological imaginary in Australia is suburban. The suburbs are the sites of economic, ecological and cultural trafficking about nature. It is argued here that the `bush', at least since the Second World War in Australia, has been more determinative of the suburban social imaginary than the `city'. In recent years, the meaning of the `bush' has transmuted from the `country' into the `wilderness', and from a valorization of the historic settlement process to myths of pure, aboriginal nature unadulterated by human imaginings and interventions. Though the meanings of the `bush' have changed in each case, its interpreter and maker is the socialized gardener of suburbia. The future of Australian nature is to be played out in the suburbs. In his famous chapter (3.82) on the revolutions engendered by the Peloponnesian War Thucydides notes that the effects of "stasis" ("faction" and "factional strife") reached even to the words people used. His overview should be translated ""Men changed the customary valuation [or" estimation"] of words in respect to deeds in judging what right was." Thucydides bases his understanding of distortions of language and understanding in revolutions on an implied idea that we need to use measures and standards in evaluating the (...) moral qualities of action. His ideas concerning revolutions and factions during the Peloponnesian War can and should be compared with Plato's, notably in "Republic" Books 1 and 8. (shrink)

The poor might always be with us but neither in ways that we imagine them nor in circumstances of their own choosing. Poverty (and its subject class, the poor) has been a persistent presence in the modern social sciences - both as ethical shadow and methodological stimulus. Throughout his self-described career as `professional storyteller of the contemporary human condition', Bauman's hermeneutical, dialectical and anthropological foci and modus operandi are impressively consistent, none more so than in his reflections on the problem (...) of poverty. An important stimulant to Bauman's ruminations on the new poor has been the work of Jeremy Seabrook and not only because Seabrook's primary foci have been the symbiotic and dialectical relationships in the production of poverty and wealth in his chronicles of the poor in contemporary capitalism, in Europe and Asia. Indeed, it is arguable that Seabrook's own vocation as freelance journalist is emblematic of the kind of sociology Bauman exhorts the salaried members of the academy to embrace for now. This article traces the lineaments of the biographical parallels, intellectual resonances and methodological (or at least epistemological) affinities shared between Seabrook and Bauman so as to tease out what a sociology of the new poor looks like in the hands of Bauman after Seabrook. The incessancy of the educational reforms of recent decades in Western countries, and their prominent association with conceptions of quality drawn from industry and commerce, tend to becloud the lack of educational substance at the heart of many of the more influential of the reform patterns. This lack betokens something of a sophisticated renaissance of the late nineteenth-century mentality of payment-by-results. Exploration of the reforms also reveals a preoccupation with performance which bypasses the central concerns of education itself. Quality, in (...) short, becomes redefined by a privative rationality, which then furnishes the conceptual arena and the predominant language for decision-making in matters educational. Writings of two influential contemporary thinkers -- MacIntyre and Lyotard -- are reviewed to illustrate the nature and significance of what the reforms have neglected. These thinkers' contrasting analyses reveal how intricate the contexts of educational policy and practice have become in the pluralist circumstances of late modernity. Where MacIntyre adopts a largely traditionalist stance and Lyotard a largely dismissive one in the face of the competing inheritances which battle for the minds and hearts of learners, this paper suggests not a middle way, but a different way. This pursues a kind of thinking which is itself educational more than political, self-critical more than adversarial. Declining the path of self-assured advocacy it concentrates instead on opening up an educational issue which is more often overlooked, or busily bypassed, than understood: What actually befalls the experience of teachers and learners in the practical conduct of education? How can that experience benefit best as teaching and learning are defensibly practised? A range of communicative rather than combative virtues is identified in this connection and their promising import is briefly explored. (shrink)

Next SectionIn just forty years, the United States has witnessed the transition in the understanding of the practice of elective abortion from that of a heinous act to that of the most common surgical procedure performed on young women. That transition was facilitated first by a set of ideas which became practices which became habitual and determinative of character and, when taken together, contributed to a tectonic shift in culture. The ideas are to be found in a set of claims—liberty (...) claims and equality claims—that elevate the will of the individual against the structures of the real world. The practices grew out of and were nourished in a matrix fashioned in the sexual revolution made effective by technological and medical advances. The practices, having been made habitual, altered the understanding of sexual activity and recast the understanding of marriage and its ends. This article delineates the defining elements that wrought such a transformation in culture. It will suggest that without a radical retransformation this culture, which has embraced the killing of the hidden vulnerable, carries the seeds of its own destruction. And it will present a response from the perspective of a woman who is by profession a philosopher and who is, by God’s grace, a Roman Catholic. (shrink)

Here we present what we believe is a novel account of what languages are, along with an axiomatically rich representation of languages and language-related data that is based on this account. We propose an account of languages as aggregates of dispositions distributed across aggregates of persons, and in doing so we address linguistic competences and the processes that realize them. This paves the way for representing additional types of language-related entities. Like demographic data of other sorts, data about languages may (...) be of use to researchers in a number of areas, including biomedical research. Data on the languages used in clinical encounters are typically included in medical records, and capture an important factor in patient-provider interactions. Like many types of patient and demographic data, data on a person’s preferred and primary languages are organized in different ways by different systems. This can be a barrier to data integration. We believe that a robust framework for representing language in general and preferred and primary language in particular – which has been lacking in ontologies thus far – can promote more successful integration of language-related data from disparate data sources. Manila is one of the world’s most fragmented, privatized and un-public of cities. Why is this so? This paper contemplates the seemingly immutable privacy of the city of Manila, and the paradoxical character of its publicity. Manila is our prime exemplar of the 21st-century mega-city whose apparent disorder discloses a coherent order which we here call ‘neo-patrimonial urbanism’. Manila is a city where poor and rich alike have their own government, infrastructure, and armies, the shopping malls are the simulacra of (...) public congregations once found in cathedrals and plazas, and where household order is matched by streetside chaos, and personal cleanliness wars with public dirt. We nominate the key characteristics of this uncanny approximation of chaotic and discordant order – a polyphonous and polyrhythmic social order but one lacking harmony – and offer a historical sociology, a genealogy that traces an emblematic pattern across the colonizing periods of its emergent urban forms into the contemporary impositions of gated zones and territories. The enduring legacy of patrimonial power to Manila is to be found in the households and on the streets that undermine and devalue public forms of social power in favour of the patriarch and his householders (now relabelled as ‘shareholders’ in ‘public companies’) at the cost of harmonious, peaceable and just public order. Such a state of affairs is not only destructive of the historic built environment of the city, especially its public parks and plazas and heritage districts, its streets, footpaths, public transport and utilities, but is directly injurious of its citizens. To address the question of Manila’s private order and public chaos is to reopen the quest for the good city as the just polis. It is also to take us beyond arguments of indigenous versus colonial forms of urbanism that are mired in nationalist and modernization ideologies respectively, and it is to reject the reductive logics of globalization arguments that Asian mega-cities are but variations of American logics of urbanism. Over the past 50 years, rock music has been the prime mover of an emergent national recording industry in Australia. In this study, which has two parts, we survey record labels, recording techniques and forms, and the music that was bought and sold. Part One narrates the emergence of modern record production, the rise of rock music, and the development of a local recording industry in Australia between 1945 and 1970. Part Two (to be published in Thesis Eleven 110) recounts (...) the rise and fall of Australian local, regional and national rock music cultures and the ebb and flow of independent labels and their labyrinthine relations to the transatlantic centre of the world-system of the rock music industry. In particular, we focus on four aspects: technological change, infrastructure, business logistics/markets, and musical production/repertoire. Since the digital revolution started eating away firstly and most conspicuously at the recording industry, the 50 years from 1945 to 1995 can now more clearly be seen as the rise and fall of rock music and its major technological form, the vinyl record. This is why we call it ‘the vinyl age’. This story has not been told in full previously, and this two-part article is a first step to bridge this gap in the historical and cultural sociology of popular music. (shrink)

There is no such thing as human culture or human cultural difference without human universality.1 (A parallel point about understanding human cultural difference was made by Donald Davidson.2) Alternatively, cultural difference is variation on human universality.2. It follows that every area of a culture manifests human universality. (Otherwise, those cultural areas would not exist.) It does not follow that all areas of culture (...) are found in all cultures. The empirical study of literature, an area of some cultures, would not exist if it did not manifest human universality. It does not follow that the empirical study of literature exists in all cultures.3. The presence of verbal art in any given culture—thus the determination of whether verbal art is found in all cultures—is dependent on the investigator's definition of the term "verbal art," not on the presence or absence of a parallel term in the culture being investigated. "Verbal art" is no different from any other term or phrase in this respect. We would not conclude that there is no color orange in a place that does not have a word parallel to "orange" or that there is no astigmatism in societies without ophthalmology—or that the English do not experience ennui or Schadenfreude.On Communicative Speech4. To understand or even isolate verbal art, we must first understand the general purposes and properties of communicative speech.5. Communicative speech aims at producing some mental alteration in an addressee. That change commonly has some direct or indirect action as its desired outcome. My aim is to produce a mental change in Wittgenstein that will result in him retrieving a plank and bringing it to me.6. Action commonly involves a set of generative principles for bodily motion or mental deliberation, a set of regulative principles for adjusting the motion or deliberation, and a set of motives for initiating the motion or deliberation.7. Communicative speech, then, commonly aims at fostering emotions and ideas or representations that, in combination, activate and specify generative principles for action in an addressee. (The emotion may be very weak—for example, a mild aversion to being uncooperative.) It may involve regulative principles as well.8. Verbal art—as I am using the phrase—also aims at producing a mental alteration in an addressee. However, that mental alteration is not, in the first place, governed by the overarching goal of effecting a particular actional outcome. Thus it is not designed to engage generative rules for action or to trigger emotions that motivate such action.9. Rather, verbal art is commonly designed to communicate regulative principles for action and to inspire emotions that do not have direct actional outcomes in the case at hand.10. There are two sorts of regulative principles that figure prominently in both ordinary life and verbal art—ethical and prudential. They constitute the thematic or didactic aspect of verbal art, an aspect noted in every major tradition of literary theory—European, South Asian, East Asian, and Middle Eastern/North African.311. Here and in other cases, individual works of verbal art need not share all universal properties or share them all in the same degree.Rather, all cultural traditions have a body of works of verbal art that, as a whole, have these properties.On Emotion12. Any sort of emotion may occur in verbal art. Indeed, verbal art involves all varieties of emotion.13. However, verbal art is not about the addressee himself or herself. Empathic emotion may be a function of situational particularity or of group identity.
