SummaryThis article documents understandings and responses to mass drug administration for the treatment and prevention of lymphatic filariasis among adults and children in northern coastal Tanzania from 2004 to 2011. Assessment of village-level distribution registers, combined with self-reported drug uptake surveys of adults, participant observation and interviews, revealed that at study sites in Pangani and Muheza districts the uptake of drugs was persistently low. The majority of people living at these highly endemic locations either did not receive or actively rejected (...) free treatment. A combination of social, economic and political reasons explain the low uptake of drugs. These include a fear of treatment ; divergence between biomedical and local understandings of lymphatic filariasis; and limited and ineffective communication about the rationale for mass treatment. Other contributory factors are the reliance upon volunteers for distribution within villages and, in some locations, strained relationships between different groups of people within villages as well as between local leaders and government officials. The article also highlights a disjuncture between self-reported uptake of drugs by adults at a village level and the higher uptake of drugs recorded in official reports. The latter informs claims that elimination will be a possibility by 2020. This gives voice to a broader problem: there is considerable pressure for those implementing MDA to report positive results. The very real challenges of making MDA work are pushed to one side – adding to a rhetoric of success at the expense of engaging with local realities. It is vital to address the kind of issues raised in this article if current attempts to eliminate lymphatic filariasis in mainland coastal Tanzania are to achieve their goal. (shrink)

Even babies have an implicit appreciation of the relationship between realistic pictures and the objects that they depict, but a mature understanding of pictures involves an explicit appreciation of how pictures work. Adults appreciate that pictures are public representations that can communicate information to other people, and that some pictures are better at doing this than others. We explore the foundations of this understanding in young children. In three experiments, using yes/no and forced-choice questions, we find that 3- and 4-year (...) olds understand that other people benefit from pictures that contain greater perceptual detail and that the more realistic the picture, the better it is as a symbolic vehicle. Here we examine the hypothesis that symbolic understanding across domains is mediated by a fundamental ‘symbolizing’ ability in young children. We tested 30 children aged 2–4 years on symbolic tasks assessing iconic and non-iconic word-referent and picture-referent understanding and administered standardised tests of symbolic play and receptive language. Children showed understanding of the symbol-referent relation earlier for pictures than written words, and performance within domains was correlated and, importantly, predicted by a marker of general symbolic ability. Performance on picture and (...) written word tasks was also unrelated to language comprehension. Thus, symbolic abilities in specific domains are underpinned by a general symbolizing ability which arises early in development. (shrink)

Bullot & Reber (B&R) put forth a design stance to fuse psychological and art historical accounts of visual thinking into a single theory. We argue that this aspect of their proposal needs further fine-tuning. Issues of transgression and coherence are necessary to provide stability to the design stance. We advocate looking to Art Education for such fundamentals of picture understanding.

We describe how a Community-Based Participatory Research (CBPR) process was used to develop a means of discussing end-of-life care needs of Deaf seniors. This process identified a variety of communication issues to be addressed in working with this special population. We overview the unique linguistic and cultural characteristics of this community and their implications for working with Deaf individuals to provide information for making informed decisions about end-of-life care, including completion of health care directives. Our research and our work with (...) members of the Deaf community strongly show that communication and presentation of information should be in American Sign Language, the language of Deaf citizens. Dennett has argued that when people interpret artifacts and other designed objects ( such as biological items ) they rely on optimality considerations , rather than on designer's intentions. On his view , we infer an item's function by finding out what it is best at; and such functional attribution is more reliable than when we depend on the intention it was developed with. This paper examines research in cognitive psychology and archaeology , and argues that Dennett's account is implausible. (...) We conclude that , quite in contrast to Dennett , intentional considerations play a crucial role in artifact hermeneutics , and even stronger , are necessary for the sake of simplicity and precision. Finally , we question Dennett's contention that the interpretation of artifacts is the same project as the interpretation of any other designed entity. (shrink)

his article develops a set of recommendations for the psychiatric and medical community in the treatment of mental disorders in response to the recently published fifth edition of the Diagnostic and Statistical Manual of Mental Disorders, that is, DSM-5. We focus primarily on the limitations of the DSM-5 in its individuation of Complicated Grief, which can be diagnosed as Major Depression under its new criteria, and Post-Traumatic Stress Disorder (PTSD). We argue that the hyponarrativity of the descriptions of these disorders (...) in the DSM-5, defined as the abstraction of the illness categories from the particular life contingencies and personal identity of the patient (e.g., age, race, gender, socio-economic status), constrains the DSM-5's usefulness in the development of psychotherapeutic approaches in the treatment of mental disorders. While the DSM-5 is useful in some scientific and administrative contexts, the DSM's hyponarrativity is problematic, we argue, given that the DSMs are designed to be useful guides for not only scientific research, but also for the education of medical practitioners and for treatment development. our goal therefore is to offer suggestions for mental health practitioners in using the DSM-5, so that they can avoid or eliminate the problems that may stem from the limitations of hyponarrativity. When such problems are eliminated, we believe that effective psychotherapeutic strategies can be developed, which would be successful in repairing the very relationships that are strained in mental disorder: the patient's relationship to herself, her physical environment, and her social environment. Hegel was a systematic philosopher, who grounded his system on a speculative logic. But his greatest philosophical contributions lie in his reflections on human culture: ethics, social and political philosophy, aesthetics, religion and the philosophy of history. This fact poses a problem for anyone who accepts it and then attempts to provide a philosophical discussion of Hegel's thought with the aim of making it available to a later age.There can be no doubt that any authentic treatment of Hegel's social and (...) political philosophy must take account of Hegel's explicit intention to ground it in his logical system of thought-determinations. But if we simply take that intention at face value, we make our appropriation of Hegel hostage to his philosophical system and speculative logic, which now are at best outdated and, though they may themselves contain some insights of lasting philosophical value, are not of nearly as much philosophical interest as Hegel's thoughts about human culture, society and history. A Hegel whose ethical, social, historical and cultural insights could be appropriated only by those who accept his speculative logical system would be a Hegel few would ever read or learn from.The other horn of the same dilemma is that those interpreters who are honest enough that they don't accept uncritically Hegel's own account of the structure of his philosophical accomplishments will inevitably be charged with doing violence to Hegel's thought, ignoring its true structure and unity. To quote a passage from Fred Beiser, which Thom Brooks uses to pillory all such Hegel scholarship: ‘We make Hegel alive and relevant, a useful contributor to our concerns; but that is only because we put our own views into his mouth. 'Evidentialism' is the conventional name (given mainly by its opponents) for the view that there is a moral duty to proportion one's beliefs to evidence, proof or other epistemic justifications for belief. This essay defends evidentialism against objections based on the alleged involuntariness of belief, on the claim that evidentialism assumes a doubtful epistemology, that epistemically unsupported beliefs can be beneficial, that there are significant classes of exceptions to the evidentialist principle, and other shabby evasions and alibis (as I take (...) them to be) for disregarding the duty to believe according to the evidence. Evidentialism is also supported by arguments based on both self-regarding and other-regarding considerations. (shrink)

Kant’s strict views on lying have been regularly cited as a reason for thinking there is something fundamentally wrong with Kantian ethics. Some of Kant’s statements here seem so excessive that most Kantians who have dealt with the topic have tried to distance themselves from them, usually claiming that they do not follow from Kant’s own principles. In this chapter, I will do a little of that, partly by questioning whether the famous example of the “murderer at the door” really (...) fits the principles Kant applies to it. By and large, however, I will argue Kant’s views about veracity are reasonable or at least defensible, if not selfevident. This is mainly because I also think some of them –especially his position in the brief, late and famous essay On a Supposed Right to Lie from Philanthropy – have been badly misunderstood. My principal aim here will be to correct that misunderstanding. (shrink)

This paper is a critical examination of one central theme in Jon Elster's Making Sense of Marx; Elster's defense of ?methodological individualism? in social science and his related critique of Marx's use of ?functional explanation?. The paper does not quarrel with Elster's claim that the particular instances of functional explanation advanced by Marx are defective; what it criticizes is Elster's attempt to raise principled, philosophical objections to this type of explanation in the social sciences. It is argued that Elster's philosophical (...) critique of functional explanation rests on a caricature of this kind of explanation, just as his critique of Marx's use of teleology in the philosophy of history rests on a caricature of the kinds of teleological claims Marx is concerned to make. The paper ends with a brief discussion of a recently published passage from Marx's notebooks of 1861?1863, where Elster claims to have found Marx explicitly criticizing capitalist exploitation as an injustice to the workers. Gandhi can serve as a valuable catalyst allowing us to rethink our philosophical positions on violence, nonviolence, and education. Especially insightful are Gandhi's formulations of the multidimensionality of violence, including educational violence, and the violence of the status quo. His peace education offers many possibilities for dealing with short-term violence, but its greatest strength is its long-term preventative education and socialization. Key to Gandhi's peace education are his ethical and ontological formulations of means-ends relations; the need to uncover root causes (...) and causal determinants and to free oneself from entrapment in escalating cycles of violence; and the dynamic complex relation between relative and absolute truth that includes analysis of situated embodied consciousness, tolerant diversity and inclusiveness, and an approach to unavoidable violence. Drawing heavily upon natural history data from the Neotropical butterfly fauna, an attempt is made to develop a model, with testable hypotheses, to account for the evolution of egg-clustering and larval gregariousness. Given the high diversity of both plant and butterfly species in the American tropics, there is a higher incidence of egg-clustering there, including some species with aposematically-colored immature stages. Emphasis is placed on the need to examine both the physical (mechanical) toughness of larval food plants for larval feeding (...) success, and the spatial structuring of food plant populations. It is argued that when food plant spatial patchiness is high, the breadth of local food plant species usage low, plant growth rates high, fed-upon structures tough, butterfly dispersal low, and reproductive potential low, egg-clustering evolves. Such ecological character states, coupled with the occurrence of food plant patches in large sizes relative to the nutritional and fecundity needs of the butterfly specialist on the species, select for egg-clustering to ensure survival of the butterfly population in that habitat. It is emphasized that the model proposed here requires considerable examination through field studies. (shrink)
