Inherent in memory is a paradox. Memory represents an attempt to fix information or an interpretation of it, an effort to freeze time into a crystalline image. But memory itself exists in time; the process of remembering destabilizes the frozen image, changing the contours of what is remembered. This paradox is embodied in the creation and subsequent cultural existence of monuments or memorials, which I define here as physical objects to which a commemorative meaning is attached. A monument is constructed (...) in order to fix a memory and its interpretation, to render them present. But this meaning is hardly fixed; it is destabilized by memory itself, which over time reinterprets the monument to fit present needs. Think, for example, of how the pyramids' significance has been transformed by New Age devotees. Such metamorphoses of a memorial's meaning hardly need a millennial time frame. As James Young has shown so powerfully, the monument to the Warsaw Ghetto uprising erected in that same city has increasingly lost its specifically Jewish referentiality, appropriated not only by the largely non-Jewish population as a reminder of Polish resistance to oppression, but even more ironically by the Palestinians in the context of their own uprising, the intifada. The goal of this review is to consider how culture impacts the socialization of emotion development in infancy, and infants’ and young children’s subsequent outcomes. First, we argue that parents’ socialization decisions are embedded within cultural structures, beliefs, and practices. Second, we identify five broad cultural frames (collectivism/individualism; power distance; children’s place in family and culture; ways children learn; and value of emotional experience and expression) that help to organize current and future research. For each frame, we discuss the impact (...) on parents’ socialization practices and infants’ subsequent outcomes relating to emotion-related experience, expression, and understanding. On December 30, 2003, the FDA announced that it will publish a rule banning sales of ephedra - a dietary supplement often utilized for weight loss, increased energy, and enhanced athletic performance - because it poses an unreasonable health risk.The ban will be issued under the auspices of the Federal Food, Drug, and Cosmetic Act and the Dietary Supplement Health and Education Act of 1994, in response to a process that began in June of 1997, when the FDA first proposed (...) a mandatory warning statement on dietary supplements containing ephedra. ObjectivesIndividuals register and react to daily sensory stimuli differently, which influences participation in occupations. Sleep is a foundational nightly occupation that impacts overall health and development in children. Emerging research suggests that certain sensory processing patterns, specifically sensory sensitivities, may have a negative impact on sleep health in children. In this study, we aimed to characterize sleep in children with and without sensory sensitivities and examine the relationship between sensory processing patterns and sleep using validated parent- and child-reported questionnaires. We (...) hypothesized that children with sensory sensitivities will exhibit more difficulties with sleep.MethodsWe recruited 22 children with sensory sensitivities and 33 children without sensory sensitivities to complete validated sleep and sensory processing questionnaires: the Children’s Sleep Habits Questionnaire, Sleep Self-Report, and Sensory Profile-2.ResultsChildren with SS had significantly more sleep behaviors reported by both parents and children compared to children with NSS. Specifically, children with SS had higher frequencies of sleep anxiety, bedtime resistance, and sleep onset delay. Spearman’s ρ correlations indicated significant positive correlations between parent- and child-reported sleep. Children with SS showed a larger association and greater variability between sleep and sensory processing compared to their peers. Significant positive correlations between parent-reported sleep behaviors and sensory sensitive and avoiding patterns were identified for both children with SS and NSS. Child-reported sleep behaviors were most strongly associated with sensitive and avoiding patterns for children with NSS and seeking patterns for children with SS.ConclusionWe present evidence that sleep is impacted for children with SS to a greater extent than children with NSS. We also identified that a child’s sensory processing pattern may be an important contributor to sleep problems in children with and without sensory sensitivities. Sleep concerns should be addressed within routine care for children with sensory sensitivities. Future studies will inform specific sleep intervention targets most salient for children with SS and other sensory processing patterns. (shrink)

To think the relationships which exist between Marxism and psychoanalysis obliges one to reflect upon the intersections between two theoretical fields, each composed independently of the other and whose possible forms of mutual reference do not merge into any obvious system of translation. For example, it is impossible to affirm—though it has often been done—that psychoanalysis adds a theory of subjectivity to the field of historical materialism, given that the latter has been constituted, by and large, as a negation of (...) the validity and the pertinence of any theory of subjectivity . Thus, no simple model of supplement or articulations is of the slightest use. The problem is rather that of finding an index of comparison between two different theoretical fields, but that, in turn, implies the construction of a new field, within which the comparison would make sense.This new field is one which may be characterized as “post-Marxist” and is the result of a multitude of theoretico-political interventions whose cumulative effect in relation to the categories of classical Marxism is similar to what Heidegger called a “de-struction of the history of ontology.” For Heidegger, this “de-struction” did not signify the purely negative operation of rejecting a tradition, but exactly the opposite: it is by means of a radical questioning which is situated beyond this tradition—but which is only possible in relation to it—that the originary meaning of the categories of this tradition may be recovered. In this sense, effecting a “de-struction” of the history of Marxism implies going beyond the deceptive evidence of concepts such as “class,” “capital,” and so on, and re-creating the meaning of the originary synthesis that such concepts aspired to establish, the total system of theoretical alternatives in regard to which they represented only limited options, and the ambiguities inherent in their constitution itself—the “hymen” in the Derridean sense—which, although violently repressed, rise up here and there in diverse discursive surfaces. It is the systematic and genealogical outline of these nuclei of ambiguity which initially allows for a destruction of the history of Marxism and which constitutes post-Marxism as the field of our current political reflection. But it is precisely in these surfaces of discursive ambiguity that it is possible to detect the presence of logics of the political which allows for the establishment of a true dialogue, without complacent metaphorization, between Marxism and psychoanalytic theory. I would like to highlight two points, which I consider fundamental, concerning these discursive surfaces. The field of emotion understanding is replete with measures, yet lacks an integrated conceptual organizing structure. To identify and organize skills associated with the recognition and knowledge of emotions, and to highlight the focus of emotion understanding as localized in the self, in specific others, and in generalized others, we introduce the conceptual framework of Emotion Understanding in Recognition and Knowledge Abilities. We then categorize 56 existing methods of emotion understanding within this framework to highlight current gaps and future opportunities (...) in assessing emotion understanding across the lifespan. We hope the EUReKA model provides a systematic and integrated framework for conceptualizing and measuring emotion understanding for future research. (shrink)

Drawing on a landscape analysis of existing data-sharing initiatives, in-depth interviews with expert stakeholders, and public deliberations with community advisory panels across the U.S., we describe features of the evolving medical information commons. We identify participant-centricity and trustworthiness as the most important features of an MIC and discuss the implications for those seeking to create a sustainable, useful, and widely available collection of linked resources for research and other purposes.

Making data broadly accessible is essential to creating a medical information commons. Transparency about data-sharing practices can cultivate trust among prospective and existing MIC participants. We present an analysis of 34 initiatives sharing DNA-derived data based on public information. We describe data-sharing practices captured, including practices related to consent, privacy and security, data access, oversight, and participant engagement. Our results reveal that data-sharing initiatives have some distance to go in achieving transparency.

Advances in technologies and biomedical informatics have expanded capacity to generate and share biomedical data. With a lens on genomic data, we present a typology characterizing the data-sharing landscape in biomedical research to advance understanding of the key stakeholders and existing data-sharing practices. The typology highlights the diversity of data-sharing efforts and facilitators and reveals how novel data-sharing efforts are challenging existing norms regarding the role of individuals whom the data describe.

These essays are meant for personal reflection and above all, for thoughtful discussion with mentors, with peers, with others throughout the health care provider community who care about acting professionally.

BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. This commentary addresses public health issues underlying homelessness and related law, policy, and advocacy options. After framing public health issues for affected individuals and the community, legal and policy approaches and related barriers are assessed. Major topics include deficits in housing availability, the role of state-based Medicaid programs, criminalization of homelessness, and the use of emergency declarations seeking to address particular issues related to homelessness in select states and localities.

A 2011 National Academies of Sciences report called for an “Information Commons” and a “Knowledge Network” to revolutionize biomedical research and clinical care. We interviewed 41 expert stakeholders to examine governance, access, data collection, and privacy in the context of a medical information commons. Stakeholders' attitudes about MICs align with the NAS vision of an Information Commons; however, differences of opinion regarding clinical use and access warrant further research to explore policy and technological solutions.

A medical information commons is a networked data environment utilized for research and clinical applications. At three deliberations across the U.S., we engaged 75 adults in two-day facilitated discussions on the ethical and social issues inherent to sharing data with an MIC. Deliberants made recommendations regarding opt-in consent, transparent data policies, public representation on MIC governing boards, and strict data security and privacy protection. Community engagement is critical to earning the public's trust.

Drug Safety Communications are used by the Food and Drug Administration to inform health care providers, patients, caregivers, and the general public about safety issues related to FDA-approved drugs. To assess patient knowledge of the messaging contained in DSCs related to the sleep aids zolpidem and eszopiclone, we conducted a large, cross-sectional patient survey of 1,982 commercially insured patients selected by stratified random sampling from the Optum Research Database who had filled at least two prescriptions for either zolpidem or eszopiclone (...) between July 1, 2012 and June 30, 2013. Among the 594 respondents, two-thirds reported hearing generally about drug safety information prior to starting a new drug, with the remaining one-third “rarely” or “never” hearing such information. Providers and pharmacists were primary sources of drug safety information. Two-thirds of zolpidem users and half of eszopiclone users reported having heard about the related DSC messages, ability to accurately identify the major factual messages was limited. Respondents reacted to new drug safety information about their sleep aids by reporting that they would want to learn about alternative ways to help them sleep and seek out more information about the safety of their specific sleeping pill. Opportunities may exist for the FDA to work with providers and pharmacies to help ensure the DSC information is more widely received and is more fully understood by those taking the affected medications. The presence of fetal cells has been associated with both positive and negative effects on maternal health. These paradoxical effects may be due to the fact that maternal and offspring fitness interests are aligned in certain domains and conflicting in others, which may have led to the evolution of fetal microchimeric phenotypes that can manipulate maternal tissues. We use cooperation and conflict theory to generate testable predictions about domains in which fetal microchimerism may enhance maternal health and those in which (...) it may be detrimental. This framework suggests that fetal cells may function both to contribute to maternal somatic maintenance (e.g. wound healing) and to manipulate maternal physiology to enhance resource transmission to offspring (e.g. enhancing milk production). In this review, we use an evolutionary framework to make testable predictions about the role of fetal microchimerism in lactation, thyroid function, autoimmune disease, cancer and maternal emotional, and psychological health.Also watch the Video Abstract. Constructions of women’s activism as social service, volunteer, or charity work contribute to the relative invisibility of these forms of activism. The author conducted field research at an affiliate office of the Susan G. Komen Breast Cancer Foundation. She analyzes how these women volunteers resist the label “activist” at the same time that they engage in activities that resemble activism. The author also examines the reasons for their resistance to the term. Her analysis shows that implicit connections between constructions of (...) activism and gender shape the extent to which volunteers think of their work either as political or as activism. In light of Komen’s heteronormative gender ideology, the author concludes by raising questions about the relationships among gender, activism, and civic participation. (shrink)

All accounts of language acquisition agree that, by around age 4, children’s knowledge of grammatical constructions is abstract, rather than tied solely to individual lexical items. The aim of the present research was to investigate, focusing on the passive, whether children’s and adults’ performance is additionally semantically constrained, varying according to the distance between the semantics of the verb and those of the construction. In a forced‐choice pointing study (Experiment 1), both 4‐ to 6‐year olds (N = 60) and adults (...) (N = 60) showed support for the prediction of this semantic construction prototype account of an interaction such that the observed disadvantage for passives as compared to actives (i.e., fewer correct points/longer reaction time) was greater for experiencer‐theme verbs than for agent‐patient and theme‐experiencer verbs (e.g., Bob was seen/hit/frightened by Wendy). Similarly, in a production/priming study (Experiment 2), both 4‐ to 6‐year olds (N = 60) and adults (N = 60) produced fewer passives for experiencer‐theme verbs than for agent‐patient/theme‐experiencer verbs. We conclude that these findings are difficult to explain under accounts based on the notion of A(rgument) movement or of a monostratal, semantics‐free, level of syntax, and instead necessitate some form of semantic construction prototype account. A minimalist or “pleonastic” ontology is supposed to provide a “cheap ontology” of languagecreated entities to serve as relatively innocuous referents for singular terms for such entities as properties, propositions, events, meanings, and fictional characters. This paper investigates the very idea of ontological minimalism, its source, and its potential applications. Certain puzzles and paradoxes arise in the idea of ontological minimalism; the article argues that these result from the fact that minimal entities divide into three different cases with importantly different (...) ontological and epistemological implications. These different analyses show that general claims of minimalism provide no guarantee that the relevant entities are merely linguistic creations that can be accepted without ontological qualms. Nonetheless, minimalism has some important applications, e.g. relative minimalism can aid in determining the relative parsimony of different theories and thereby help demonstrate why certain forms of eliminativism regarding “ordinary objects”, fictional characters, etc. are misguided. The paper begins with a historical perspective on the operating environment in Bhopal, the events surrounding the accident, then discusses an international situation audit examining internal strengths and weaknesses, and external opportunities and threats faced by Union Carbide at the time of the accident. There is a discussion (...) of management of the various interests involved in international public relations and ethical issues. A review of the financial ratio analysis of the company prior and subsequent to the accident follows, then an examination of the second tragedy of Bhopal — the tragic failure of the international legal system to adequately and timely compensate victims of the accident.The paper concludes with recommendations towards public policy, as well as a call for congressional action regarding international safety of U.S. based multinational operations. Phenomenology and analytic philosophy were born out of the same historical problem---the growing crisis about how to characterize the proper methods and role of philosophy, given the increasing success and separation of the natural sciences. A common 18th and 19th century solution that reached its height with John Stuart Mill’s psychologism was to hold that the while natural science was concerned with “external, physical phenomena”, philosophy was concerned with “internal, mental phenomena”, and thus proceeded by turning our observational gaze inward (...) at the mind, rather than outward towards the world. Both Husserlian phenomenology and early analytic philosophy grew from dissatisfaction with psychologism, and figures from both traditions developed relentless criticisms of psychologism, beginning with Brentano and G.E. Explores a postmodern criticism of P. S. Churchland's claims regarding materialism. Materialism is classically understood to be the philosophical position which holds that matter is the fundamental reality of the world, and so neurobiological explanations can be said to be materialistic. Neurobiological explanations of behavior are used increasingly in the place of psychological explanations. This trend is indicative of the rise in popularity of materialism. Churchland is one of the intellectual leaders in the modern manifestation of materialism. She is a (...) self-proclaimed materialist and "neurophilosopher." In order for Churchland to maintain her materialist position, she must assume a transparency in scientific method, an assumption which is a legacy of Enlightenment philosophy. However, many postmodern philosophers including M. Heidegger and H. G. Gadamer have questioned this assumed transparency of method.
