This paper reports on an interpretive research project which examines the feasibility of implementing social audit within the general medical practice setting. The study aims to communicate patients' voices to aid evaluation of the potential contribution of social audit to the public health sector and also addresses particular conceptual problems which arise when attempting to implement social audit within this environment. The fieldwork focuses on one general health practice in Lanarkshire (in southern central Scotland). Consultative focus group discussions and individual (...) interviews were carried out with a sample of twenty two patients. Patients were most concerned with health service organisational and delivery issues. Overall, the results suggest patients are enthusiastic about the ideas and process of the social audit. Patients were able to demonstrate a capacity for grasping accountability issues and balanced reasoning. There was an acute awareness of the constraints which exist in the public services. Patients addressed the broad rights and responsibilities issues, and highlighted the possible pitfalls of relying on a representational mechanism for ensuring their voices were heard. The study concludes that an involved and negotiative dialogue process of implementing social audit could provide beneficial understanding and ideas to the organisation which may be further researched. Invoking its social function can explain why we find a certain functional trait or institution only if we can identify a mechanism whereby the playing of the function connects with the explanandum. That is the main claim in the missing-mechanism critique of functionalism. Is it correct? Yes, if functional explanation is meant to make sense of the actual presence of the trait or institution. No, if it is meant to make sense of why the trait or institution is resilient: why (...) we can rely on it to survive various contingencies. The lesson? Social functionalism should be taken, and may have been taken by its founders, as a programme for explaining resilience. (shrink)

Technological advances in veterinary medicine have produced considerable progress in the diagnosis and treatment of numerous diseases in animals. At the same time, veterinarians, veterinary technicians, and owners of animals face increasingly complex situations that raise questions about goals of care and correct or reasonable courses of action. These dilemmas are frequently controversial and can generate conflicts between clients and health care providers. In many ways they resemble the ethical challenges confronted by human medicine and that spawned the creation of (...) clinical ethics committees as a mechanism to analyze, discuss, and resolve disagreements. The staff of the North Carolina State University Veterinary Hospital, a specialty academic teaching institution, wanted to investigate whether similar success could be achieved in the tertiary care veterinary setting. In common-sense morality, certain special obligations loom large. These are duties which are laid upon agents, be they individuals or groups, in virtue of their distinctive identities, relationships or histories: because of who they are, how they are linked to others or what they have done in the past. The particularistic basis of these obligations means that no one but the agent in question is engaged by such a duty. It is that agent's alone.These special obligations include duties towards oneself, (...) towards one's dependants and towards those to whom one has made certain commitments. In each case they prescribe partisan treatment. The beneficiary is to enjoy a benefit which may not be especially important in the global scheme of things, and which may even be obtained at the cost of others’ being deprived of benefits that are equally or more important in some cosmic sense. Over the last century, and especially since the publication of the Belmont Report in 1978, respect for persons, as exemplified by respect for autonomous decision-making, has become a central tenet in the practice of medicine. The authority of cognitively competent adults to make their own healthcare decisions is enshrined in both law and practice in most advanced industrialized nations. The right to consent to or to refuse medical interventions is virtually absolute, but is contingent on the provision of materially relevant (...) information about the benefits and burdens or risks of the proposed treatment as well as the freedom from coercion by others, especially healthcare personnel. This power also extends to the kinds, amounts and details of the proposed intervention, including the option to decline to hear anything, if one so chooses, assuming that this is a rational choice. This respect transfers to other competent individuals who are authorized as surrogates to decide for those who have temporarily or permanently lost the capacity to make their own healthcare decisions. In 2001, the algebraico-tree-theoretic simplicity hierarchical structure of J. H. Conway’s ordered field ${\mathbf {No}}$ of surreal numbers was brought to the fore by the first author and employed to provide necessary and sufficient conditions for an ordered field to be isomorphic to an initial subfield of ${\mathbf {No}}$, i.e. a subfield of ${\mathbf {No}}$ that is an initial subtree of ${\mathbf {No}}$. In this sequel, analogous results are established for ordered exponential fields, making use of a slight generalization of (...) Schmeling’s conception of a transseries field. It is further shown that a wide range of ordered exponential fields are isomorphic to initial exponential subfields of $$. These include all models of $T$, where ${\mathbb R}_W$ is the reals expanded by a convergent Weierstrass system W. Of these, those we call trigonometric-exponential fields are given particular attention. It is shown that the exponential functions on the initial trigonometric-exponential subfields of ${\mathbf {No}}$, which includes ${\mathbf {No}}$ itself, extend to canonical exponential functions on their surcomplex counterparts. The image of the canonical map of the ordered exponential field ${\mathbb T}^{LE}$ of logarithmic-exponential transseries into ${\mathbf {No}}$ is shown to be initial, as are the ordered exponential fields ${\mathbb R})^{EL}$ and ${\mathbb R}\langle \langle \omega \rangle \rangle $. Argues i) that part of what it is to understand what is being asked, when we ask whether something is good, is being able to distinguish stopping points in a series of "Why?" questions, and ii) that this ability explains how we can reason from observable facts to conclusions about value.

There is a growing trend among some physicians, psychiatrists, bioethicists, and other mental health professionals not to treat physician-assisted suicide (PAS) as suicide. The grounds for doing so are that PAS fundamentally differs from other suicides. Perhaps most notably, in 2017 the American Association of Suicidology argued that PAS is distinct from the behavior that their organization seeks to prevent. This paper compares and contrasts suicide and PAS in order to see how much overlap there is. Contrary to the emerging (...) view that emphasizes their differences, I argue that there is significant overlap such that we ought not to separate PAS into its own category, making it diverge from how we think about and address suicide more generally. I start by examining several prominent theories of suicide and argue that PAS fits squarely within them. I then examine several apparent differences between PAS and other kinds of suicide and argue that these differences are merely apparent or they do not justify treating PAS as a fundamentally different kind of thing from suicide. The philosophies of Merleau-Ponty and the later Wittgenstein are shown to yield a common position opposing 'realist' attempts to reduce appearance, sense, and meaning to perception-independent objects and relations. Their 'Gestalt Philosophy' thus constitutes a new form of 'anti- realism'.

Historical analysis and policy making often require counterfactual thought experiments that isolate hypothesized causes from a vast array of historical possibilities. However, a core precept of Jervis's “systems thinking” is that causes are so interconnected that the historian can only with great difficulty imagine causation by subtracting all variables but one. Prediction, according to Jervis, is even more problematic: The more sensitive an event is to initial conditions (e.g., butterfly effects), the harder it is to derive accurate forecasts. Nevertheless, if (...) awareness of system effects can help forecasters better calibrate their probability estimates of whether or not certain events will come to pass, systems thinkers who are pessimistic about prediction are diluting their confidence too much. The challenge is a meta-cognitive one: thinking systematically about when to engage in systems thinking; and weighing the costs and benefits of using simple or complex heuristics in policy environments that can shift suddenly from quiescence to turbulence.
