The commodification of health care, particularly primary care, presents challenges to care and knowledge development. The purpose of this study is to examine how nurses perceive and develop their knowledge in a commodified context. A mixed‐methods study was conducted that included a closed‐question survey and in‐depth interviews with nurses in public primary care in Catalonia. There were 104 valid responses to the questionnaire and 10 in‐depth interviews. The main findings of the survey were related to workload and limited (...) time for nursing care. Six themes emerged from the in‐depth interviews: (1) limited time for nursing, (2) feelings of burnout, (3) awareness of patient and family satisfaction, (4) organizational factors that favor nurses' needs, (5) organizational factors that hinder nurses' needs, and finally (6) public administration requirements. Participants perceive excessive workload and time constraints and feel that this affects their nursing care and their physical and mental health. However, nurses purposefully use knowledge patterns to cope with the problems associated with commodification. Nurses have multidimensional, contextualized, and integrated knowledge that allows them to optimize their care based on the needs of their patients. This research examines many challenges related to nursing practice and the nursing discipline and opens the door for further research that encompasses all areas of nursing. (shrink)

What ethically justifies the provision of invasive and irreversible treatments to minors? In this commentary, I examine this question in response to Moti Gorin's article “What Is the Aim of Pediatric ‘Gender‐Affirming’ Care?,” which critiques autonomy‐based arguments for justification of gender‐affirming care in minors. Minors generally lack sufficient autonomy to make significant medical decisions or major life decisions. For this reason, parents are generally their decision‐makers, working with medical professionals to choose treatments that serve the best interests of the minor. (...) Medical care in minors is justified by beneficence, not autonomy, and this should be no different for gender‐affirming care. This severely undermines autonomy‐based arguments for provision of gender‐affirming care to minors. Given the lack of conclusive evidence for benefit, the nature of the treatment, and the fact that gender dysphoria in minors resolves spontaneously in most cases, there is presently insufficient justification for provision of such care to minors. Primary care presents distressful moral problems for nurse practitioners (NPs) who report frustration, powerlessness, changing jobs and leaving advanced practice. The purpose of this grounded theory study was to describe the process NPs use to manage moral problems common to primary care. Twenty-three NPs were interviewed, commenting on hypothetical situations depicting ethical issues common to primary care. Coding was conducted using a constant comparative method. A theory of maintaining moral integrity emerged consisting of the phases of encountering (...) conflict, drawing a line, finding a way without crossing the line, and evaluating actions. The NPs varied in their awareness and the discord encountered in conflict, and in clarity, flexibility and justification of the line drawn. A critical juncture occurred when NPs evaluated how well integrity had been maintained. Some experienced no distress while others experienced self-doubt, regret, outrage and frustration at external constraints, and attempted to reconcile through avoiding, convincing themselves, and compensating. In Europe, welfare state provision has been subjected to 'market forces'. Over the last two decades, the framework of economic competitiveness has become the defining aim of education, to be achieved by new managerialist techniques and mechanisms. Lane et al.'s proposal that psychotherapeutic change comes about through memory reconsolidation is compelling; however, the model would be strengthened by the inclusion of predictions regarding additional factors that might influence treatment response, predictions for improving outcomes for non-responsive patients, and a discussion of how the proposed model might explain individual differences in vulnerability for mental health problems.

The purpose of the study is to reveal the features of the influence of empathy on the development of the sensory sphere of the younger school student in music lessons at school. The article focuses on the specifics of the formation of empathy, its connection with musical material. Scientific novelty lies in the substantiation of the purposeful development of the empathic sphere of children in music lessons, in the specification of its factors and criteria. As a result, the stages of (...) development of empathy, its connection with the life and sensory experience of the student, his behavior, personal characteristics, as well as the guiding role of joint creative activity with the teacher, which helps to develop the experience of the student's musical experiences, affects the formation of his feelings, develops the ability to recognize non-verbally expressed emotions by music, is revealed. The first half of this review article on Locke on primary and secondary qualities leads up to a fairly straightforward reading of what Locke says about the distinction in Essay II.viii, one that, in its general outlines, represents a sympathetic understanding of Locke’s discussion. The second half of the paper turns to consider a few of the ways in which interpreting Locke on primary and secondary qualities has proven more complicated. Here we take up what is sometimes called (...) the Berkeleyan interpretation of Locke, the understanding of Locke’s resemblance thesis, and Locke’s views of qualities and their relationship to powers. I discuss a learning outcome of the Western Carolina University, Department of Philosophy and Religion, which focuses on a student’s development and pursuit of a meaningful, thriving, well-lived life, as a corrective to the poverty of existential reflection in the academy. We achieve this Socratic goal via a targeted series of assignments throughout the student’s education, a required pro-seminar on the topic of human flourishing, and other elective courses. The self-reflective, narrative assignments are designed to help students develop their own (...) tentative position on a flourishing life, consider whether and how they are pursuing that vision, and articulate meaningful ways to integrate potential vocations with a fulfilling existence. Students are introduced to these questions in introductory classes; take a pro-seminar analyzing various positions on ‘the good life’ drawn from numerous philosophical, religious, and literary texts, with a research paper assignment critically evaluating one position of their choice ; and, in their final year, revisit their earlier ‘meaning assignments,’ examining retrospectively their academic career and personal development, and prospectively their postbaccalaureate life and career. I conclude with some suggestions for programs keen to incorporate a similar outcome into their own curriculum. (shrink)

The capability approach to distributive justice, as defended by Amartya Sen and Martha Nussbaum, represents perhaps the most influential recent attempt to reconcile the competing demands of liberty and equality. Specifically, capability theorists have claimed that their insistence on the universal cultivation of a set of capabilities for basic human "functionings" is fully consistent with a liberal neutrality commitment. Their reason is that these capabilities are, like Rawls's primary goods, rational to want "whatever else one wants." This article suggests, (...) in contrast, that the capability approach fails to satisfy the neutrality requirement endorsed by both Sen and Nussbaum. It further suggests that the non-neutral character of the approach reflects its Rawlsian lineage, and raises serious doubts about the coherence of Rawls's account of primary goods. (shrink)

In this paper I argue that, in Leibniz’s mature metaphysics, primary matter is not a positive constituent which must be added to the form in order to have a substance. Primary matter is merely a way to express the negation of some further perfection. It does not have a positive ontological status and merely indicates the limitation or imperfection of a substance. To be sure, Leibniz is less than explicit on this point, and in many texts he writes (...) as if primary matter were a positive constituent of a substance. It seems to me, however, that the view most in keeping with the thrust of his mature philosophical system is that captured by a striking remark of 1695: “Materia rerum est nihilum: id est limitatio [The matter of things is nothing: that is, limitation].” This becomes especially apparent in texts showing that Leibniz’s conception of primary matter corresponds to his conception of creaturely limitation. I start by discussing the notion of primary matter in the scholastic tradition. I then show that although Leibniz places the scholastic terminology of primary matter at a crucial juncture of his metaphysics, he thinks of primary matter in a way which significantly deviates from earlier scholastic views. I conclude that despite his adoption of distinctive terminology of Aristotelian scholasticism, instead of holding a broadly Aristotelian concept of primary matter as the ultimate subject of inherence, Leibniz thinks of primary matter according to a Neoplatonic blue-print in which matter is non-being, privation, mere absence of perfection. I argue that Davidson's conception of motivating reasons as belief-desire pairs suggests a model of normative reasons for action that is superior to the orthodox conception according to which normative reasons are propositions, facts, or the truth-makers of such facts.

Review of Primary Ousia: An Essay on Aristotle's Metaphysics Z and H, by Michael J. Loux (Cornell University Press: 1991).

The paper argues for realism in quantum mechanics. Specifically, the formalism of quantum mechanics should be understood as giving a complete description of quantum situations. When it is understood in this way, traditional primary properties of matter can be seen as similar to traditional secondary properties, though at a different level.

Fourteen newly commissioned essays trace the historical development of the distinction between primary and secondary qualities, which lies at the intersection of issues in metaphysics, epistemology, and philosophy of perception. 'Primary and Secondary Qualities' focuses on the age of the Scientific Revolution, the 'locus classicus' of the distinction, but begins with chapters on ancient Greek and Scholastic accounts of qualities in an effort to identify its origins. Michael J. Loux here presents a fresh reading of two of the most important books of the Metaphysics, Books Z and H, in which Aristotle presents his mature theory of primary substances. Focusing on the interplay of Aristotle's early and late views, Loux maintans that the later concept of ousia should be understood in terms of a theory of predication that carries interesting implications for contemporary metaphysics. Loux argues that in his first attempt in identifying ousiai in the Categories, (...) Aristotle encountered a set of ontological problems which he wrestled with again in Metaphysics Z and H. In the Categories, where the primary realities are basic subjects of predication construed in essentialist terms as things falling under natural kinds, familiar particulars are the primary ousiai. In subsequent works, Aristotle holds that since familiar particulars come into being and pass away, they must be composites of matter and form; and in Metaphysics Z and H, he explores the implications of this insight for the search for ousia. Maintaining that the substantial forms of familiar particulars are the primary ousiai, the later Aristotle interprets forms as predicable universals rather than as particulars, each uniquely possessed by a single object. The new system of axioms we propose is based on the foundational theory of De Giorgi et al. Scuola Normale Superiore di Pisa, Preprints di Matematica 26: 1 (1996) slightly modified. In that paper (which is dedicated to a new axiomatic framework for mathematics, informatics and logic) the authors use two kinds of primitive notions: relations and qualities. Since their system is based on the distribution paradigm, they start from distinction. We propose to shift the perspective and to start from (...) unity and then from within unity to pass to distinction; to this end we apply ideas of Lesniewski, Nijhoff International Philosophy Series 44 (1992). We introduce only one kind of entity as a primitive notion, namely relations, and treat qualities as articulations of relations. The new concept of "primary relation" permits the introduction of a dynamic, non-standard form of identity, which we hope will find application in various fields where self-referential structures are required. With the exception of solidity, Locke's list of primary qualities matches his list of ideas of "divers senses," that is, ideas that are perceived in multiple sensory modalities. I argue that for these ideas, the fact that they are robust in our sensory experience in a way that single-modality ideas are not provides the main reason for taking them to be ideas of primary qualities. Solidity, however, is taken as primary because it is ineliminable from experience in (...) a way that other ideas are not, and because the multiply-sensed ideas are all tied to it. Since solidity is experienced through touch, Locke is thus offering us an account of primary quality perception grounded in our embodied interaction with the world. (shrink)

The primary objects of hearing are sounds: everything we hear we hear by hearing a sound. (This claim differs from Berkeley’s that we hear only sounds and from Aristotle’s that we only hear sounds.) Colored regions are primary objects of sight, and pressure resistant regions are primary objects of perception by touch. By definition, the primary objects of perception are physical. The properties of the primary objects of perception are exactly the properties sense-datum theories attribute (...) to sense-data. Indirect Realism holds that awareness of sense-data (or something similar) mediates our perception of primary objects. Direct Realism denies this. The question when the perception of a primary object, such as parts of the surfaces of a hat and coat, is thereby the perception of a non-primary object, such as a person, is independent of the disagreement between Direct and Indirect Realism. This study investigated the internal mechanism of the relationship between primary school students’ peer relationships and their performance in the Chinese language and literature. We constructed a chain mediation model, focused on the mediation effects of parental involvement and the sense of autonomy, on the correlation between peer relationships and performance in Chinese language scores. A questionnaire survey was conducted among 1,503 students in grades 4–6, and their parents, in three cities in Jiangsu Province. The result indicated the following: (...) there was a significant positive correlation between primary school students’ peer relationships, parental involvement, sense of autonomy, and the level of Chinese language scores; parental involvement and the sense of autonomy, respectively, mediate the relationship between peer relationships and Chinese language scores; primary school students’ parental involvement and sense of autonomy play a chain-like mediating role in the relationship between their peer relationships and Chinese language scores. The research results provide a reference for exploring the educational strategies of primary school students’ Chinese literacy. (shrink)

Once dismissed as vestigial organelles, primary cilia have garnered the interest of scientists, given their importance in development/signaling, and for their implication in a new disease category known as ciliopathies. However, many, if not all, “cilia” proteins also have locations/functions outside of the primary cilium. These extraciliary functions can complicate the interpretation of a particular ciliopathy phenotype: it may be a result of defects at the cilium and/or at extraciliary locations, and it could be broadly related to a (...) unifying cellular process for these proteins, such as polarity. Assembly of a cilium has many similarities to the development of other polarized structures. This evolutionarily preserved process for the assembly of polarized cell structures offers a perspective on how the cilium may have evolved. We hypothesize that cilia proteins are critical for cell polarity, and that core polarity proteins may have been specialized to form various cellular protrusions, including primary cilia. (shrink)

While Chile's partial decriminalization of abortion in 2017 was a long overdue recognition of women's sexual and reproductive rights, nearly four years later the caseload remains well below expectations. This pattern is the product of standing barriers in access to abortion‐related health services, especially at the primary care point of entry. This study seeks to identify and describe these barriers. The findings presented here were obtained through a qualitative, exploratory study based on 19 semi‐structured interviews with relevant actors identified (...) through non‐random sampling and snowballing techniques. Coding was inductive and complemented by semantic content analysis. The authors find that the key barriers in primary care to accessing legal abortion are unfamiliarity with the law, insufficient practitioner training, intersectoral discrimination, and the stigma surrounding abortion. They conclude that the government needs to exercise its constitutional mandate as guarantor of public health and act promptly to safeguard and guarantee the abortion rights of Chilean women. In this paper, I argue that Book II, Chapter viii of Locke' Essay is a unified, self-consistent whole, and that the appearance of inconsistency is due largely to anachronistic misreadings and misunderstandings. The key to the distinction between primary and secondary qualities is that the former are, while the latter are not, real properties, i.e., properties that exist in bodies independently of being perceived. Once the distinction is properly understood, it becomes clear that Locke's arguments for it are simple, (...) valid and (in one case) persuasive as well. (shrink)

John Rawls based his theory of justice, in the work of that name, on a ‘Kantian interpretation’ of the status of human beings as ‘free and equal’ persons. In his subsequent, ‘political rather than metaphysical’ expositions of his theory, the conception of citizens of democracies as ‘free and equal’ persons retained its foundational role. But Rawls appealed only to Kant’s moral philosophy, never to Kant’s own political philosophy as expounded in his 1797 Doctrine of Right in theMetaphysics of Morals. I (...) argue here that the structure of Kant’s political philosophy, with its categories of the innate right to freedom, private acquired right and public right, can clarify the relationship between Rawls’s two principles of justice and his scheme of basic liberties and primary goods. Since the Fall of 1993, at the Centre Interdisciplinaire de Recherche sur l'Apprentissage et le D/span>veloppement en /span>ducation of the Universit/span> du Qu/span>bec /span> Montr/span>al, two mathematicians and one philosopher have collaborated to design and develop a research project involving philosophy, mathematics and sciences. Previous observations in the classroom had led the researchers to realize that, within the school curriculum, children like some subject matters and dislike others. Most of them usually succeed in arts, physical education and language arts, but (...) many have difficulties in succeeding in mathematics. Why? On the one hand, as Matthew Lipman advocates, the school curricula are not sufficiently "meaningful" for children. On the other hand, some studies in the field of mathematics suggest that there are myths and prejudices about mathematics in primary schools and that the school system is partly responsible for this. Indeed, the school system does not invite children to express emotions in class about mathematics nor does it favor creativity. It does not allow dialogue among peers about mathematical concepts and problems, nor the construction of mathematical knowledge by the students themselves. (shrink)

Certain problems with standard two-dimensional semantics are addressed and cases in which these problems arise explored. In such cases the primary intension cannot be univocally mapped in one and only one indexical world, thus standard two-dimensional semantics cannot efficiently address the problems presented. Subsequently, a modified model is presented which leads these problems to be averted in the replicated cases. This modified model admits primary intensions that are not univocally mapped. Introduction in chapter viii of book ii of An Essay Concerning Human Understanding, John Locke provides various putative lists of primary qualities. Insofar as they have considered the variation across Locke's lists at all, commentators have usually been content simply either to consider a self-consciously abbreviated list (e.g., "Size, Shape, etc.") or a composite list as the list of Lockean primary qualities, truncating such a composite list only by omitting supposedly co-referential terms. Doing the latter with minimal judgment (...) about what terms are co-referential gives us the following list of eleven qualities (in the order in which they appear in this chapter of the Essay): solidity, extension, figure, mobility, motion or rest, number, bulk, texture, motion, size, and situation. Perhaps surprisingly given the attention to the primary/secondary distinction since Locke, Locke's primary qualities themselves have received little more than passing mention in the bulk of the subsequent literature. In particular, no discussion both offers an interpretation of Locke's conception of primary qualities and makes sense of Locke's various lists as lists of primary qualities. A central motivation for this paper is the idea that these two tasks are not independent. (shrink)

Demands for public participation in technical decision-making are currently high on the agenda of Science & Technology Studies. It is assumed that the democratisation of technical decision-making processes generally leads to more socially desirable and acceptable outcomes. While this may be true in certain cases, this assumption cannot be generalised. I will discuss the case of the so-called ‘South African AZT debate’. The controversy started when President Thabo Mbeki, after reading some scientific papers on the toxicity of AZT, decided to (...) bar the use of the drug in the public health sector as a means to reduce the transmission of HIV from mothers to children. While the scientific mainstream accepts the effectiveness of AZT in reducing the risk of vertical HIV transmission, a few maverick scientists reject the clinical evidence and argue that the risks of using AZT by far outweigh its benefits. Based on various textual sources and using the ‘Periodic Table of Expertises’ developed by Collins and Evans, Mbeki’s expertise at the time of his intervention into the technical question whether AZT is a medicine or a poison can be classified as primary source knowledge. It is shown that this type of expertise is insufficient for technical decision-making. Mbeki’s primary source knowledge legitimated his presentation of the claims of maverick scientists as a serious contribution to the debate—with tragic consequences for tens of thousands of babies.Keywords: Primary source knowledge; Expertise; Technical decision-making; South Africa; AZT; AIDS. In this paper I will argue that Crispin Wright’s defence of the claim that the truth about intention is judgement-dependent is unstable because it can serve also to establish that the truth about shape is judgement-dependent, thereby violating his constraint that in developing the distinction between judgement-independent and judgement-dependent subject matters we have to be driven by the assumption that colour and shape will fall on different sides of the divide.

This paper criticises Miranda Fricker's account of the primary harm of testimonial injustice as a kind of epistemic objectification, where the latter is understood on the model provided by Martha Nussbaum's influential analysis of sexual objectification and where it is taken to involve the denial of someone's epistemic agency. I examine the existing objections to Fricker's account of the primary harm, criticising some while accepting the force of others, and I argue that one of Fricker's own central examples (...) of testimonial injustice in fact offers the basis of a particularly telling objection. While Fricker's other critics have mostly concluded that we need to look at alternative theoretical resources to offer an account of the primary harm of testimonial injustice, I aim to show that this is premature; both Fricker and her critics have underestimated the resources provided by Nussbaum's analysis of objectification when offering an account of the primary harm, and something very much in the spirit of Fricker's account survives the objections. Descartes’ derivation of the primary qualities of matter and their role in explaining observed physical phenomena are briefly reviewed. The lesson drawn from Descartes’ methodology of explanation is that we ought to aim to reduce complex phenomena to simple unifying principles and conceptual primitives. Three proposed solutions to the apparent paradoxes in contemporary quantum physics (primarily associated with the notion of entanglement) are briefly compared with lessons taken from Descartes. It is argued that further research in this field should (...) provide criteria for selecting modifications to the standard (largely Cartesian) conceptual scheme, along the lines of either visualisable causality or the physical spatial separability of all material objects. The aim of this article is to give an account of the distinction between primary and secondary qualities in a way which allows the distinction a useful place in an explanation of scientific enquiry. this is done by modifying certain of locke's criteria for primacy, and by showing that this procedure has certain advantages over keith campbell's account of the distinction. in particular, i argue that primary qualities cannot be specified in a theory-neutral way, and that this has (...) important consequences for an account of scientific observation and the relation between the scientific and everyday images of the world.
