Malaysia had its first four patients with COVID-19 on 25 January 2020. In the same week, the World Health Organization declared it as a public health emergency of international concern. The pandemic has since challenged the ethics and practice of medicine. There is palpable tension from the conflict of interest between public health initiatives and individual’s rights. Ensuring equitable care and distribution of health resources for patients with and without COVID-19 is a recurring ethical challenge for clinicians. Palliative care aims (...) to mitigate suffering caused by a life-limiting illness, and this crisis has led to the awareness and urgency to ensure it reaches all who needs it. This paper asks which free logic a Fregean should adopt. It examines options within the tradition including Carnap’s (1956) chosen object theory, Lehmann’s (1994, 2002) strict Fregean free logic, Woodruff’s (1970) strong table about Boolean operators and Bencivenga’s (1986, 1991) supervaluational semantics. It argues for a neutral free logic in view of its proximity towards explaining natural languages. However, disagreeing with Lehmann, it claims a Fregean should adopt the strong table based on Frege’s discussion on generality. Supervaluation uses strong table (...) and aims to give it a semantic justification. However, supervaluation is in turn justified by convention or mental experiments, which Lehmann argues as inadequate. The paper proposes a new justification of supervaluation based on sense and two-dimensional semantics. The resulting model, coined Supervaluational Neutral Free Logic (SNFL), resolves many conflicts between Lehmann and Bencivenga while staying close with Frege’s discussions about non-denotation. It also provides new insights into the relations among truth, logical truth, and supervaluated truth (or supertruth, for short). (shrink)

Russell had two theories of names and one theory of description. Logically proper names are Millian names, which have only denotation but no connotation. Ordinary names are not genuine names but disguised definite descriptions subject to quantificational analyses. Only by asserting that ordinary names are definite descriptions could Russell motivate his theory of description to solve three problems for Millian names, namely, Frege’s puzzle, empty reference and negative existentials. Critics usually discuss Russell’s theories of names and his theory of description (...) separately. This paper takes a new perspective and presents a dilemma for the overall project, arguing that it is hard to be a Russellian about names coherently. The central issue is whether contextualisation is semantic or pragmatic in nature, an issue very much alive in contemporary debates. This paper traces Russell’s ambiguity on this matter back to his conception of the roles of knowledge by acquaintance and knowledge by description in naming. (shrink)

This paper examines to what extent Davidsonian truth-theoretic semantics can give an adequate account for empty names in natural languages. It argues that the prospect is dim because of a tension between metaphysical austerity, non-vacuousness of theorems and empirical adequacy. Sainsbury (2005) proposed a Davidsonian account of empty names called ‘Reference Without Referents’ (RWR), which explicates reference in terms of reference-condition rather than referent, thus avoiding the issue of existence. This is an inspiring account. However, it meets several difficulties. First, (...) there is no non-vacuous, interpretive truth-condition available for any T-sentence containing an empty name, because by stipulation there is no way to compose a true atomic sentence using an empty name in RWR. The absence of an interpretive truth-condition implies the absence of an interpretive reference-condition. It also entails empirical inadequacy to support an interpretation because the related propositional attitude can never be truly satisfied. These considerations cast doubt on whether truth is the proper vehicle to articulate meaning. This paper suggests that we can alleviating the problem by employing truth-conditions varying across contexts of utterance, rather than just contexts of evaluation. The result is a two-dimensional model-theoretic approach to empty names. It involves a new function coined counterfactual reference and the account is compatible with both descriptivism and Millianism. (shrink)

User intent classification is a vital component of a question-answering system or a task-based dialogue system. In order to understand the goals of users’ questions or discourses, the system categorizes user text into a set of pre-defined user intent categories. User questions or discourses are usually short in length and lack sufficient context; thus, it is difficult to extract deep semantic information from these types of text and the accuracy of user intent classification may be affected. To better identify user (...) intents, this paper proposes a BERT-Cap hybrid neural network model with focal loss for user intent classification to capture user intents in dialogue. The model uses multiple transformer encoder blocks to encode user utterances and initializes encoder parameters with a pre-trained BERT. Then, it extracts essential features using a capsule network with dynamic routing after utterances encoding. Experiment results on four publicly available datasets show that our model BERT-Cap achieves a F1 score of 0.967 and an accuracy of 0.967, outperforming a number of baseline methods, indicating its effectiveness in user intent classification. The counseling process involves attention, emotional perception, cognitive appraisal, and decision-making. This study aimed to investigate cognitive appraisal and the associated emotional processes when reading short therapists' statements of motivational interviewing (MI). Thirty participants with work injuries were classified into the pre-contemplation (PC,n= 15) or readiness stage of the change group (RD,n= 15). The participants viewed MI congruent (MI-C), MI incongruent (MI-INC), or control phrases during which their electroencephalograms were captured. The results indicated significant Group × Condition effects in the (...) frontally oriented late positive complex (P600/LPC). The P600/LPC's amplitudes were more positive-going in the PC than in the RD group for the MI congruent statements. Within the PC group, the amplitudes of the N400 were significantly correlated (r= 0.607–0.649) with the participants' level of negative affect. Our findings suggest that the brief contents of MI statements alone can elicit late cognitive and emotional appraisal processes beyond semantic processing. (shrink)

Background: Frailty is a natural consequence of the aging process. With the increasing aging population in Mainland China, the quality of life and end-of-life care for frail older people need to be taken into consideration. Advance Care Planning has also been used worldwide in long-term facilities, hospitals and communities to improve the quality of end-of-life care, increase patient and family satisfaction, and reduce healthcare costs and hospital admissions in Western countries. However, it has not been practiced in China. Research objective: (...) This study aimed to evaluate the effectiveness of a modified Advance Care Planning intervention in certainty of end-of-life care, preferences for end-of-life care, quality of life concerns, and healthcare utilization among frail older people. Research design: This study used a quasi-experimental design, with a single-blind, control group, pretest and repeated posttest approach. Participants and research context: A convenience sample of 74 participates met the eligibility criteria in each nursing home. A total of 148 frail older people were recruited in two nursing homes in Zhejiang Province, China. Ethical considerations: The study received ethical approval from the Clinical Research Ethics Committee, the Faculty of Medicine, and The Chinese University of Hong Kong, CREC Ref. No: 2016.059. Findings: The results indicated the Advance Care Planning programme was effective at increasing autonomy in decision making on end-of-life care issues, decreasing decision-making conflicts over end-of-life care issues, and increasing their expression about end-of-life care. Discussion: This study promoted the participants’ autonomy and broke through the inherent custom of avoiding talking about death in China. Conclusion: The modified Advance Care Planning intervention is effective and recommended to support the frail older people in their end-of-life care decision in Chinese society. (shrink)

BackgroundIn the Canadian Alliance for Healthy Hearts and Minds (CAHHM) cohort, participants underwent magnetic resonance imaging (MRI) of the brain, heart, and abdomen, that generated incidental findings (IFs). The approach to managing these unexpected results remain a complex issue. Our objectives were to describe the CAHHM policy for the management of IFs, to understand the impact of disclosing IFs to healthy research participants, and to reflect on the ethical obligations of researchers in future MRI studies.MethodsBetween 2013 and 2019, 8252 participants (...) (mean age 58 ± 9 years, 54% women) were recruited with a follow-up questionnaire administered to 909 participants (40% response rate) at 1-year. The CAHHM policy followed a restricted approach, whereby routine feedback on IFs was not provided. Only IFs of severe structural abnormalities were reported.ResultsSevere structural abnormalities occurred in 8.3% (95% confidence interval 7.7–8.9%) of participants, with the highest proportions found in the brain (4.2%) and abdomen (3.1%). The majority of participants (97%) informed of an IF reported no change in quality of life, with 3% of participants reporting that the knowledge of an IF negatively impacted their quality of life. Furthermore, 50% reported increased stress in learning about an IF, and in 95%, the discovery of an IF did not adversely impact his/her life insurance policy. Most participants (90%) would enrol in the study again and perceived the MRI scan to be beneficial, regardless of whether they were informed of IFs. While the implications of a restricted approach to IF management was perceived to be mostly positive, a degree of diagnostic misconception was present amongst participants, indicating the importance of a more thorough consent process to support participant autonomy.ConclusionThe management of IFs from research MRI scans remain a challenging issue, as participants may experience stress and a reduced quality of life when IFs are disclosed. The restricted approach to IF management in CAHHM demonstrated a fair fulfillment of the overarching ethical principles of respect for autonomy, concern for wellbeing, and justice. Mental toughness (MT) is a key psychological variable related to achievement in performance domains and perseverance in challenging circumstances. We sought to understand the lived experiences of mentally tough high-altitude mountaineers, focusing primarily upon decisions to persevere or abort summit attempts. Phenomenological interviews were conducted with 14 mountaineers including guides, expedition leaders, and doctors (Mage = 44 years). A content analysis was employed to identify key themes in the data. Participants emphasized the importance of MT in extreme environments and described (...) rational, flexible, and vigilant decision-making. Turning around without summiting was the toughest decision reported, with recognition of the thin line between persevering and overstretching. In contrast to much MT literature, mountaineers accepted limits, demonstrated restraint, and sacrificed personal goals to aid others. Costly perseverance was also reported with some mountaineers described as “too tough”: overcompetitive, goal-obsessed, and biased decision-makers. These findings revealed both benefits and dangers of MT in mountaineering. (shrink)

In this paper I offer a new interpretation of the philosophical method of the "Meno." In the opening discussion of the dialogue, Plato introduces a restriction on answers in dialectical inquiry, which I call the Dialectical Requirement (DR). The DR is applied twice in the "Meno," in different ways (75d5-7, 79d1-3). In the first section of the paper, I argue that the two applications of the DR represent the beginning and end of dialectic. This shows that dialectical inquiry starts from (...) our linguistic competence with the name of the property we investigate, and ends only when we have an account saying what is common to and explanatory of the bearers of that property. Dialectic begins in our ordinary ability to speak and think about the world, and ends in genuine grasp of the underlying causes of nature. In the second section, I describe the resources of linguistic competence, and their role in dialectical progress. Our linguistic competence with the name of a property enables us to make a wide variety of statements about the property and its bearers in ordinary discourse. In dialectic, these ordinary statements act as a portfolio in which the property under investigation is presented to us writ large, through its instances, types, species, etc. We seek to develop an account that says what is common to, and explanatory of the phenomena in the portfolio. When an account is inconsistent with one of the things we tend to say, this demands revision either in the account, or in the portfolio of statements. In this way, the process by which we develop our account also helps to organize and revise the statements in our portfolio so that they and the account ultimately form a coherent, explanatory body of statements. Research now provides participants greater indications of genetic risk for disease, even for conditions incidental to the research study. Given this development, should such information also be disclosed to the family of research participants? There has been some indication at the national level that genetic risk information can be disclosed to participants' families; however, limited attention has been given to returning research results to family. Thus, we have also incorporated the discussion surrounding the disclosure of genetic risk discovered in the (...) clinic (e.g., genetic testing). A number of important questions are examined: Should genetic research results be provided to family? Are there differences between clinical and research findings that would prevent research results from being disclosed to family? Who should make the disclosure, if in fact it is done at all? We conclude by noting that the return of results is increasingly accepted as technology permits the discovery of more and more medically useful data. However, debates of whether results should be returned to participants must first be settled before moving to familial disclosure. (shrink)

Intelligent design, though unnecessary in the study of biological evolution, is essential to the study of cultural evolution. However, the intelligent designers in question are not deities or aliens but rather humans going about their lives. In this paper I wish to examine in detail the arguments which Spinoza uses in a very brief section of the Ethics, the lemmas following Proposition 13 of Part II. My aim in this analysis will be twofold: to attempt a preliminary sketch of the nature of a physical system in Spinoza’s view, and to clarify what Spinoza means by speaking of certain items as “individuals.” At least a partial fulfillment of the first aim is a necessary condition for the (...) second, since most of what Spinoza has to say about individuation in the Ethics and elsewhere is expressed in terms of individual bodies. His own solution of the mind-body problem entails that there will be correlative statements true at the ideational level; but, unfortunately, he usually leaves such correlations to his reader. I use the term “ideational” herein in preference to “mental” to avoid any dualistic connotation. Ideational and extensional levels for Spinoza refer to a dualism of denoting complexes, but not to a dualism of objects denoted by these. (shrink)

Hayes began writing these poems (...) straight after Trump's remarkable triumph in the 2016 presidential election, and they race to keep pace with the rapidly metastasizing effects of the Trump phenomenon, from his relentless radicalization of the modes and mores of political speech ("Newshounds ponder the tweets of a bullhorn"), his continuous assault on American democratic institutions, and his singular success in marshalling a newly cohesive political constituency seduced by the reactionary allure of resurgent white nationalism or what Hayes caustically calls "a mandate for whiteness, virility, sovereignty / stupidity" (AS: 38).2 As the phrase "mandate for whiteness" suggests, integral to the story of Trump's ascendancy in American Sonnets is another story, a story about race and social authority in the time of Black Lives Matter when blackness remains "the color of this country's current threat / advisory" (AS: 10). At home and abroad the appalling incandescence of Hayes's contemporary American political moment is reflected by the names of its black victims, names like Trayvon Martin, Michael Brown, Tamir Rice, and, perhaps most explosively of all, George Floyd, whose May 2020 police killing on a Minneapolis street reprised such a familiar configuration of racial power that Hayes had already glimpsed its spectral outline two years earlier: "A [End Page 60] brother has to know how to time travel & doctor / himself when a knee or shoe stalls against his neck" (AS: 77).3Without compromising the intensity of Hayes's focus on this contemporary social emergency, I want in what follows to expand our sense of the historical and metaphysical scene of his writing by reading it as an imaginative response to a larger and more fundamental question: What does it mean to live a black life in an antiblack world? I'm moved to do so by the double register of his writing vividly exemplified by the penultimate sonnet in the sequence which memorializes "All the black people I'm tired of losing / All the dead from parts of Florida, Ferguson / Brooklyn, Charleston, Cleveland, Chicago / Baltimore, where the names alive are / Like the names in graves" (AS: 81). What's remarkable, to my mind at least, about the phrase "the names alive are / like the names in graves" is its fusion of a directly contemporary perception of the precariousness of black existence within a culture of white supremacy (the perception that every black life could share the social fate of Tamir Rice at any particular moment) with an encompassing metaphysical vision of blackness as the embodiment of social death and the continuing afterlife of slavery. Paying close attention to several of Hayes's poems, I will trace this double register to explore a number of ideas central to his thinking in American Sonnets including the foundational role of antiblackness in organizing American reality for both black and white identities, the dialectical relationship between antiblackness and the libidinal economy of white supremacy, and the hope of imagining new possibilities of life capable of contesting the antiblack conditions of contemporary social existence.4 In doing so I will place particular emphasis on Hayes's development of a distinctively dialectical and dialogical style of poetic address that reimagines lyric subjectivity as a drama of racial becoming focused on the relationship between black social death and the culture of white supremacy.5Black Social Death and the Libidinal Economy of WhitenessBefore continuing let me briefly develop three ideas above which help shape my reading of American Sonnets: the social life of social death, the continuing afterlife of slavery, and what Hayes calls in my epigraph "the [End Page 61] invention of blackness." Both Watts and Beit-Hallahmi are enthusiastic about attachment theory as an important contribution to the psychology of religion, but they raise very different criticisms regarding other aspects of the book. I respond to Beit-Hallahmi by defending my assertion that a scientific approach to psychology of religion need not lead to the conclusion, nor rest on the premise, that the beliefs under study are ontologically false. I argue further that this "veridicality trap" has deep roots in prevailing, deeply mistaken assumptions about (...) how the mind/brain works, and show how an evolutionary approach clearly exposes such reasoning as fallacious and offers a powerful, alternative perspective on the problem. In response to Watts, I defend evolutionary psychology against the charge that it is not sufficiently empirical, and argue that the computational cognitive approach he prefers should be seen as complementary to an evolutionary approach rather than an alternative to it.
