The U.S. Food and Drug Administration's rationale for supporting the development and approval of BiDil for heart failure specifically in black patients was based on under-powered, post hoc subgroup analyses of two relatively old trials , which were further complicated by substantial covariate imbalances between racial groups. Indeed, the only statistically significant difference observed between black and white patients was found without any adjustment for potential confounders in samples that were unlikely to have been adequately randomized. Meanwhile, because the accepted (...) baseline therapy for heart failure has substantially improved since these trials took place, their results cannot be combined with data from the more recent trial amongst black patients alone. There is therefore little scientific evidence to support the approval of BiDil only for use in black patients, and the FDA's rationale fails to consider the ethical consequences of recognizing racial categories as valid markers of innate biological difference, and permitting the development of group-specific therapies that are subject to commercial incentives rather than scientific evidence or therapeutic imperatives. This paper reviews the limitations in the scientific evidence used to support the approval of BiDil only for use in black patients; calls for further analysis of the V-HeFT I and II data which might clarify whether responses to H-I vary by race; and evaluates the consequences of commercial incentives to develop racialized medicines. We recommend that the FDA revise the procedures they use to examine applications for race-based therapies to ensure that these are based on robust scientific claims and do not undermine the aims of the 1992 Revitalization Act. It is arguably the case that, in the wake of the Spanish Civil War and World War II, a new and more aerial form of spatial perception came into prominence, one in which (...) something or someone might reasonably be expected to fall out of the sky. Such a form of perception, vertical rather than horizontal in its orientation, is often experienced as an uneasiness directed at falling in both its physical and its metaphysical senses. That is, there is both a literal fear of falling from a height and a spiritual angst about falling, generally through no agency of one’s own, from one station in life to another.Within the postwar French tradition, 1 the conjunction of these two forms of falling is nowhere better articulated than in Georges Perec’s semi-autobiographical novel W or the Memory of Childhood. Most authors who discuss willpower assume that everyone knows what it is, but our assumptions differ to such an extent that we talk past each other. We agree that willpower is the psychological function that resists temptations – variously known as impulses, addictions, or bad habits; that it operates simultaneously with temptations, without prior commitment; and that use of it is limited by its cost, commonly called effort, as well as by the person's skill at executive functioning. However, accounts are (...) usually not clear about how motivation functions during the application of willpower, or how motivation is related to effort. Some accounts depict willpower as the perceiving or formation of motivational contingencies that outweigh the temptation, and some depict it as a continuous use of mechanisms that interfere with re-weighing the temptation. Some others now suggest that impulse control can bypass motivation altogether, although they refer to this route as habit rather than willpower.It is argued here that willpower should be recognized as either or both of two distinct functions, which can be calledresolveandsuppression. Resolve is based on interpretation of a current choice as a test case for a broader set of future choices, which puts at stake more than the outcome of the current choice. Suppression is inhibiting valuation of (modulating) and/or keeping attention from (filtering) immediate alternatives to a current intention. Perception of current choices as test cases for broader outcomes may result in reliable preference for these outcomes, which is experienced as an effortlesshabit– a successful result of resolve, not an alternative method of self-control. The defining characteristics anticipated for any prospective psychophysically neutral language are explored in this essay through the analysis and comparison of two previous approaches. The idea of a psychophysically neutral language was first articulated byWolfgang Pauli in the context of the dual-aspect theory of mind and matter that he developed with C.G. Jung. The first approach discussed is George Spencer Brown's Laws of Form. An overview is given, followed by a review of the critical responses and extensions of the (...) work, particularly Francisco Varela's attempt to use it to formalize biological autopoiesis and Louis Kaufman's development of Brownian algebra. The mathematical basis of Spencer Brown's calculus and its philosophical assumptions and implications are then investigated. The second approach discussed is Marie-Louise von Franz's Number and Time, an investigation of number archetypes through which she continued the inquiry begun by Jung and Pauli. The central tenets of her work are summarized and critically evaluated, and a comparison with Spencer Brown's work is carried out. Finally, implications are drawn for any future attempt to formulate a psychophysically neutral language. (shrink)

ABSTRACT The list of proposed addictions has recently grown to include television, videogames, shopping, day trading, kleptomania, and use of the Internet. These activities share with a more established entry, gambling, the property that they require no delivery of a biological stimulus that might be thought to unlock a hardwired brain process. I propose a framework for analyzing that class of incentives that do not depend on the prediction of physically privileged environmental events: people have a great capacity to coin (...) endogenous reward; we learn to cultivate it, and, where it is entrapping, to minimize it, by managing internally generated appetites for it. The basic method of cultivating endogenous reward is to learn cues that predict when best to harvest the reward that has been made possible by the growth of these appetites. This hedonic management occurs in the same motivational marketplace as the instrumental planning that seeks environmental goods in the conventional manner, and presumably obeys the same laws of temporal difference learning; but these laws are no longer limiting. Furthermore, instrumental contingencies often provide the most productive structure for hedonic management as well, for reasons that I discuss; but the needs of hedonic management create incentives both to pursue instrumental goals in a suboptimal manner and to avoid noticing how the hedonic incentive affects this pursuit. The result is the apparent irrationality that is often observed in process addictions. (shrink)

Pursuing Trayvon Martin: Historical Contexts and Contemporary Manifestations of Racial Dynamics explores the historical implications of the fatal shooting of the unarmed black teen, Trayvon Martin, by George Zimmerman, in 2012. The book telescopes various themes that are important to a broad market, including race, masculinity, racial profiling, racist stereotyping, black youth and police violence, and racism.

While there is no denying the relevance of ethical knowledge and analytical and cognitive skills in ethics consultation, such knowledge and skills can be overemphasized. They can be effectively put into practice only by an ethics consultant, who has a broad range of other skills, including interpretive and communicative capacities as well as the capacity effectively to address the psychosocial needs of patients, family members, and healthcare professionals in the context of an ethics consultation case. In this paper, I discuss (...) how emotion can play an important interpretive role in clinical ethics consultation and why attention to the role of defense mechanisms can be helpful. I concentrate on defense mechanisms, arguing first, that the presence of these mechanisms is understandable given the emotional stresses and communicative occlusions that occur between the families of patients and critical care professionals in the circumstances of critical care; second, that identifying these mechanisms is essential for interpreting and managing how these factors influence the way that the “facts” of the case are understood by family members; and, third, that effectively addressing these mechanisms is an important component for effectively doing ethics consultation. Recognizing defense mechanisms, understanding how and why they operate, and knowing how to deal with these defense mechanisms when they pose problems for communication or decision making are thus essential prerequisites for effective ethics consultation, especially in critical care. In this article, I argue that African-American philosophy emerges from a socio-existential context where persons of African descent have been faced with the absurd in the form of white racism (This paper is a substantially revised version on an earlier article. See Yancy, G. (2011). African-American Philosophy through the Lens of Socio-Existential Struggle. Philosophy & Social Criticism, Volume 37: 551–574). The concept of struggle, given the above, functions as both descriptive and heuristic vis-à-vis the meaning of African American philosophy. Expanding (...) upon Charles Mills’ concept of non-Cartesian sums, I demonstrate the inextricable link between Black lived experience, struggle, and the morphology of meta-philosophical assumptions and philosophical problems specific to African-American philosophy. Because of the philosophical pretensions of white Western philosophy, with it claims to universal truth and objective knowledge, the particularity of African-American philosophical concerns with questions of embodiment and race is often deemed ersatz or non-philosophical. In this article, I argue that whiteness as the transcendental norm is productive of a form of ignorance endemic to Western philosophical practices that are myopic and hegemonic. Finally, African-American philosophy is theorized as a gift, as a critical counter-narrative that can be deployed to fissure Western philosophy’s narcissism. (shrink)

The history of philosophy is replete with attempts at invoking rationality as a means of directing and even subduing human desire and emotion. Understood as that which moves human beings to action, desire and emotion come to be associated with human freedom and rationality as a means of curbing that freedom. Plato, for instance, takes for granted a separation between thought and action that drives a wedge between our rational ability to exercise self-discipline and the free expression of desire and (...) emotion. Hobbes, on the other hand, replaces our internal ability for rational self-control with the external authority of the political State. So long as freedom and control are pitted against one another, human beings are incapable of attaining a symbiosis of these two elements of human action so essential to realizing true democracy. Unlike Plato, John Dewey sees in democracy the greatest potential for individual and social life. The purpose of this dissertation is to demonstrate how the educational philosophy of Dewey, which culminates in an educational aesthetic, appeals to and makes the most of the symbiosis of freedom and self-control, emotion and reason. Dewey’s educational aesthetic not only offers an alternative to traditional methods of education, but also demonstrates how the goal of a democratic way of life is made feasible by means of intelligently guided self-discipline—a form of self-control guided by intelligence that is not a constraint upon freedom, but instead embodies greater opportunity for freedom. I trace the basis for this synthesis, in the social-political and pedagogical principles of John Locke and Jean-Jacques Rousseau. Both Locke and Rousseau offer educational theories that begin turning our attention toward the essential partnership required of rationality and emotion. Dewey's educational aesthetic is then considered as a response to alienating forms of education that continue to pit control and freedom against one another, and which thwart the intellectual and emotional development necessary for autonomy and democratic forms of social organization. The dream of a community of philosophers engaged in inquiry with shared standards of evidence and justification has long been with us. It has led some thinkers puzzled by our mathematical experience to look to mathematics for adjudication between competing views. I am skeptical of this approach and consider Skolem's philosophical uses of the Löwenheim-Skolem Theorem to exemplify it. I argue that these uses invariably beg the questions at issue. I say ?uses?, because I claim further that Skolem shifted his (...) position on the philosophical significance of the theorem as a result of a shift in his background beliefs. The nature of this shift and possible explanations for it are investigated. Ironically, Skolem's own case provides a historical example of the philosophical flexibility of his theorem. Our suspicion ought always to be aroused when a proof proves more than its means allow it. Something of this sort might be called ?a puffed-up proof?. Wegner makes an excellent case that our sense of ownership of our actions depends on multiple factors, to such an extent that it could be called virtual or even illusory. However, two other core functions of will are initiation of movement and maintenance of resolution, which depend on our accurate monitoring of them. This book shows that will is not an imponderable black box but, rather, an increasingly accessible set of specific functions.

Liberal theology has wanted to revise and reinterpret Christian faith in conformity with history, reason, a scientific worldview, and a sophisticated grasp of the significance of symbol, analogy, and metaphor in the lives of religious practitioners. Eliminating both supernaturalism and anthropomorphism, liberal theology would make Christianity more intellectually tenable, rescuing it from the literalist hands of fundamentalists, without collapsing it into the equally literalist arms of atheism.The extreme difficulty of evading the either-or dilemma of choosing either fundamentalism or atheism landed liberal theology in a catch-22: either it rejected the intellectually untenable elements of belief, in which case it eliminated just those beliefs that constituted its appeal to popular piety, or it retained certain supernaturalistic and anthropomorphic beliefs, understood as “symbolic,” in which case it forfeited intellectual intelligibility and any appeal to sophisticated moderns. The first version is far too weakened for ordinary American Christians, and the second is still too religious for the masters of suspicion and their followers.It is for this reason that the history of American religious thought reveals a grave irony that complicates the hopes of pragmatic historicists, the protagonists of this superbly clear and rich, deep dive into authors that most readers of this journal will have read, but perhaps not have synthesized as thoroughly as has Demian Wheeler. That irony, as Wheeler knows, is that the more intellectual changes pragmatic historicists make to traditional Christian belief, the more they help to make unbelief viable. The question then arises as to whether pragmatic historicism is simply a half-way house on the road to an atheistic destination. Or, if the word “atheism” sounds too old-fashioned, like something that went out with steamships and girdles, then “agnosticism.” Or, if “agnosticism” still smacks too much of stale debates, why not “pantheism,” as recently revived by Mary-Jane Rubenstein?The detailed development, the verbal bravura, and the measured knowledge in this book are astonishingly impressive. Wheeler’s initial thesis is that liberal theology, particularly of the early Chicago School, and American pragmatism, particularly C. S. Peirce, William James, and John Dewey, provide important resources for meeting the challenges posed by the rise of historical consciousness. Sheila Davaney has designated this tradition “pragmatic historicism,” and Wheeler aims to show its compelling import for the future of theology itself. A good deal of the book is devoted to exposition and defense of the views of: Ernst Troeltsch, James, Dewey, George Burman Foster, Shirley Jackson Case, Shailer Matthews, and Gerald Birney Smith, down to Delwin Brown, Sheila Davaney, and William Dean (Wheeler’s teachers at Iliff), along with Gordon [End Page 98] Kaufman and Sallie McFague. In the last chapter, Wheeler draws expertly on the theological views of Bernard Loomer, Michael Hogue, Donald Crosby, and Wesley Wildman. He does not simply call for a pragmatic historicist approach to theology, but he executes one as well, showing in more detail than his predecessors what it means for such topics as religious diversity (chapters 3 and 4), the nature of authority (chapter 5), the meaning of critical realism, fallibilism, and an altered correspondence theory of truth (chapters 6 and 7), and the doctrine of God (chapter 8). The proliferation of ethics committees and ethics consultation services has engendered a discussion of the issue of the expertise of those who provide clinical ethics consultation services. In this paper, I discuss two aspects of this issue: the cognitive dimension or content knowledge that the clinical ethics consultant should possess and the practical dimension or set of dispositions, skills, and traits that are necessary for effective ethics consultation. I argue that the failure to differentiate and fully explicate these dimensions contributes (...) to the confusion over the issue of expertise and fuels, at least partly, the controversies about expertise (or authority) in ethics and the legitimacy of the use of ethical knowledge in clinical ethics consultation.
